Abstract

Objective:Toexaminetheincidenceofperipheralnerveinjurywithin90daysofalimb
traumadiagnosisinpatientswhohavepresentedtothehospitaloroutpatientclinic.
Design:Thisstudyisaretrospective,descriptivestudythatusesthe1998MarketScan
CommercialClaimsandEncountersDatabase(TheMEDSTATGroup)totrackperipheral
nerveinjuriesinextremitytrauma.WeselectedoursamplebyusingICD9codesforlimb
traumaduringthefirst9mosof1998.Nerveinjurieswithin90daysafterlimbtraumawere
themainoutcomemeasure.Finally,wereporthowtheratesoftraumaandaccompanying
nerveinjurieswerecontrastedbygenderandagegroup,usingunivariateandbivariate
statistics.AlldataanalyseswereconductedusingStata9.0statisticalsoftware.
Results:Outof16millioninsuredsinthedatabase,220,593(1.4%)werediagnosedwithlimb
trauma.Eightythreepercentofthepatientswerelessthan55yrsold,and50%weremale.
Thetotalincidenceofnerveinjurieswithin90daysofupperorlowerlimbtraumawas
1.64%.Thetypeofextremitytraumawiththehighestincidenceofnerveinjurywithin90
daysofthediagnosiswasacrushinjuryat1.9%.Approximately50%ofoursamplewas
selectedbecauseofadislocation,whichhadanassociatednerveinjuryprevalenceof
1.46%.
Conclusions:Whenlookingatthepopulationsampled,ratesforperipheralnerveinjuryin
peopleincurringlimbtraumaarelow.Crushinjuriesseemtohavethehighestrateof
associatednerveinjury.Furtherstudiesareneededtoobserveoutcomesforpeoplewith
nervedamageaftertrauma.
