Classroom Rules
1. Stay on task to complete assignments

3. Be respectful: check your body language, voice and words.

Classroom Expectations
Be Ready: Be seated and prepared with class supplies to learn when the tardy bell alarms.
Get Permission: Raise your hand for permission to ask questions or make requests, or to
get out of your seat for any reason.
No Food: Dont drink beverages, eat food, or chew gum in class.
Follow Lab Safety Rules: Keep everyone safe!
Be a Student of Character: Show trustworthiness, respect, responsibility, fairness,
caring, & citizenship.
Follow CHAMPS Daily Expectations: Conversation, Help, Activity, Movement,
Participation, & Success.

ASSERTIVE DISCIPLINE STEPS for failure to follow the Classroom


Rules
The following Discipline Plan will be in effect at all times to guarantee your child and the
students in my classroom the excellent, safe educational environment they deserve. Every
student can and will be successful at Sequoia Middle School. Please read the following
information together to help your child reach the expected level of success.

Verbally remind student of appropriate behaviors.


Issue Responsibility Notice to serve time after school (15 minutes).
Behavior log entry on ATLAS and Parents/Guardian contact.
Issue a level 2 conduct referral to the office for chronic behaviors.
Level 3 conduct referral escorted to the office for severe behaviors.
Step 5: Parents/student/teachers/administrator conference will determine a
behavior plan.

Further discipline actions


Administrator assigned lunch and/or after school detention.
Students can be removed from lab activities and alternate assignments may be given.
Parents can be charged for any broken, damaged, or missing equipment.
Suspension from school and/or student returns with parent for administrative
conference.
Referral to Student Study Team, Transition Class.
Alternative program (Transition Class)

CLASS SUPPLIES
Writing utensil, agenda, and Interactive Science Notebook (ISN). I will provide each
student with an ISN, it will be your students responsibility to have their ISN every day for
class. Students do have the option to leave the ISN in class or take it home.

STUDENT HELP
I am available during lunch, before school (7:30am-8:00am), or after school (2:30pm3:00pm) for help (excluding some Tuesdays and Thursdays which are reserved for staff
meetings). Students can get missing assignments before/after classes, before school, after
school or during lunch (get a pass in advance from Ms. Milazzo). It is the responsibility of
students and parents to notify the office and teachers of anticipated absences in advance, so
make-up assignments can be arranged and picked up.

A = 90% 100%
B = 80% 89%
C = 70% 79%
D = 60% 69%
F = 50% 59%

Students will use Interactive Science Notebooks


in the classroom daily for notes and important
content information. The majority of the course
work will be found in the ISN. Students must
follow the objectives, guidelines, and expectations
for their ISN.

50% of a course grade is based on summative assessments of learning aligned to the


common Core Standards as organized in the FUSD scope and sequence.
40% of a course grade is based on formative tasks: mini-tasks, classwork, homework, and
short formative quizzes.
10% of a student grade is based on positive academic habits: class participation,
agenda/binder check, and returning a parent-signed progress report.
Character and citizenship should be reflected in the citizenship mark on the report card.
Students must have their Interactive Science Notebooks every day.

GRADING PRACTICES
Assignments/projects/labs are graded by points, participation points, completion points,
and/or group/partner points at the discretion of the teacher.
Absent/missing/incomplete assignments will be accepted. Students have 5 days to make up
absent work and turn in late work. Additional extensions and accommodations can be
made, but should be discussed with Ms. Milazzo prior to the 5 day deadline.
Students will be allowed to retake summative assessments after actively participating in reteaching opportunities outside of the regularly scheduled class time. Students must set up
an appointment with Ms. Milazzo before school, after school, or at lunch.
Students will receive two to three homework assignments a week to reinforce learning.
Parents/Students can monitor grades using Atlas.
Group/class assignments may be video recorded for grading purposes. Students are
expected to participate in group/class discussions for points toward their final grade.
