3. Nature of Event:

Set withdrawn to attend Boiler Tube Leakage.

4. Observations: At about 20:25 hrs ID fan loading suddenly increased. Drum level and Deaerator
level continuously dropped. Immediately load reduced to 110 MW & level made
normal. After checking, heavy abnormal sound was found in economiser zone right
side. One by one coal mills withdrawn & TA set hand tripped at 21:08 hrs.
5. Analysis After physical inspection of Boiler, heavy abnormal sound was observed in Economiser
zone above

Economiser hopper right side. RHS steam cooled wall tube leakage

observed first above Economiser hopper.

6. Works carried out by maintenance section: Right hand side steam cooled wall at 33 meter
level, tube
number 65 having leakage. Nearest tubes number 63, 64, 66 observed secondary
damaged and also rear side steam cooled wall tube number 132,133,134 observed secondary
damaged & number 134,135 supply tubes also observed secondary damaged. Damaged tubes
replaced by new ones & provided spool piece. Shields and cassettes provided at Economiser coil.
7. Works carried out for the restoration by operation :

8. Recommendations to avoid reoccurrence :

a) Short term NIL
b) Long term Second pass steam cooled wall panels will be inspected/repaired/replaced
considering life of tubes history during Annual Overhaul.
