3. Nature of Event: Turbine hand tripped due to tripping of both BFPs (A&B), 3 mills (D,E,F), feeders
of all mills and drum level showing minus 400 mmWC.
4. Observations:

At 11:45 hrs DCS system hanged. At 13:13 hrs during restoration of DCS system,
both BFPs (A&B), 3 mills (D, E, F), feeders of all mills tripped resulting in drum level
showing minus 400 mmWC. Therefore Turbine hand tripped at 13:15 hrs.

5. Analysis Remote #2 controller (primary) of HFC make DCS U#2 CSTPS Chandrapur got suddenly
hanged on dt.21/08/08 at 11.45 hrs and stopped functioning of all control loops which
were working under Remote #2 controller and created emergencies for running Unit.
Under such circumstances controller changeover did not take place and no indication,
alarm appears on monitor. Hot standby Remote #2 controller changeover made
manually.During this changeover, Drum level and all other critical parameters were
showing errotic hence Turbine hand tripped.

8. Recommendations to avoid reoccurrence :


a) Short term In every Unit S/D, Hot standby & running controller changeover trial will be
taken.
b) Long term Matter refered to M/S ILK & M/S HFC for solution.
