ADENOMATOUS

POLYP
•Gastrointestinal Polyp is
defined as a mass that
protrudes into lumen of the
gut. They are usually
asymptomatic and have a
potential for malignant
transformation.
• Adenomatous polyps are neoplasms
that arise from mucosal epithelium.
They are composed of neoplastic
epithelial cells that have migrated
to surface & accumulated beyond
needs of replacing the sloughed
epithelium.
• It is commonly seen in older
people above 65 yrs age with
slight male predominance.

• They are usually asymptomatic


but may manifest with rectal
bleeding.
Gross Appearance

• Single or multiple they are


mostly located in rectosigmoid
region and can be detected by
digital examination or
sigmoidoscopy.
• They vary from small
pedunculated adenomas to
large sessile adenomas.

• Grossly they can be classified


as tubular, villous &
tubulovillous.
Microscopic appearance
• Tubular adenoma consists of
closely packed epithelial
tubules which may be uniform
or irregular and excessively
branched.

• The tubules are embedded in


fibrovascular stroma similar to
normal lamina propria.
• Most tubular adenomas show little
epithelial dysplasia.

• Villous adenoma is composed of


epithelium in form of thin fingerlike
processes like villi of small
intestine supported by a core of
fibrovascular stroma. Villous
adenomas more frequently contain
foci of carcinoma.
• Polyps with more than 25% and
less than 75% villous
architecture are called
tubuovillous adenomas
