servant nursing with this vulnerable

properties/qualities of this population that
lends them to being a vulnerable
population. Work with mentor to identify
outlets/resources for the patients

Work with mentor to identify factors that have been

noticed in her practice with this population.

Working closely with the mentor and speaking with

the patients asking how to better serve them
and ways to improve upon their care

Speak with social work to identify any outlying

factors that may lead to the population being
underserved.
Speak with many patients and families to help
identify areas that can be improved upon to
better serve this population.

Evidence of Accomplishments
(To be filled in by end of Semester)

Outcomes:
(To be filled in by end of
Semester)

Spoke with patients with respect; spent time with patients

actively listening at all times. Completed 1 on 1 room visits
with the patients to talk about their history, family life, and
fears they may have about current situations. Held hands
with patients, offered prayer and reflection time with every
patient. Escorted patients who had not had family visits, to
meals and activities, and spent extra time for 1 to 1 visits if
they were open to it. Treated every patient with highest
respect, maintaining privacy at all times.

Developed stronger sense of

servant leadership and
integrated this into daily
practice. Became more
comfortable with prayer and
quiet time with patients.

Spoke in depth with all the staff members about the population
at hand, and their experiences with working with them.
Spoke with the patients in depth about their experiences with
having been in the service and any problems they have had
since being out. Spoke with patients about their experience
with working with case management and social work in the
past, and whether they believed they got any benefit from
that. Spoke with family members of patients who expressed
their frustrations and concerns about problems with care that
their family members have received; i.e. problems with
receiving benefits, problems not getting patients into
outpatient rehab for alcohol abuse and PTSD, lack of
support for PTSD, and lack of assistance with housing.
Spoke with social work about resources available to patients
for housing/rehab/counseling.

Learned more about the

population at hand and what
concerns both the patients
and families have that should
be addressed upon
admission. These patients
may need more assistance
with disposition after
discharge, and PTSD should
be addressed at every
encounter. Family members
should be encouraged to
participate in PTSD
counseling. Learned more
about flashbacks and how to
deal with them.
