Health Promotion Session Plan

Overall Health Content targeted: Communication skills Specific Component targeted: Self-image and communication
Client or Group Profile: Teenagers, grades 9-12 male and female

Session Activities
Opening activity Individual self-assessment
Activity Objective: The student will respond to variety of questions asked by the facilitator to get a better understanding of the individual and
become comfortable.
Time frame: 15-20 minutes
Activity Description: The trained professional and the client will ask each other different questions to get to know each other and become
comfortable with each other.
Materials needed: Pen or pencil and paper

Main activity Collage illustration


Activity Objective: The student will become familiar with various models for the development of depression. Students will demonstrate, through a
collage poster, there illustrations of their own life experiences in words and pictures to express what they believe contributes to mental wellness or
illness.
Time frame: 25-30 minutes
Activity Description: Individually the students will create a collage illustrating their own life experiences in words and pictures to express what they
believe contributes to mental wellness or illness, depending on the ticket they had picked. Cutting out pictures from the magazines and newspapers
that they think defines mental wellness or illness depending on which one the student picks.
Materials needed: Magazines, newspapers, scissors, tape, glue, poster paper, markers, colored pencils, and scrap paper.

Additional activity Discussion from collages


Activity Objective: After participating in the collage illustration activity the students will do a presentation on their posters and why they chose the
illustrations they did and begin in discussion to get an understanding of what the student think of a positive illustration and a negative illustration.
Time frame: 10-25 minutes

Activity Description: Upon bringing the class back together, after creating collage illustration the trained professional will present to them the pros
and cons of self-images. As well as discuss strategies used to help a vulnerable friends of student at risk for depression. The students will break into
two groups one that is mental wellness and on the positive side and the second was mental illness which is on the negative side. Then will have a
discussion why the students had chosen the illustrations that they did for their character.
Materials needed: Journal notebook (take notes in), collage posters to present

Question # 1: What do you hope to accomplish in these sessions?
Desired Client Response: Reduce or eliminate the negative issues with self-image
Question # 2: How do you feel you can better your self-image after todays session?
Desired Client Response: The client will have a better understanding of positive self-images and will be able to develop the skills to adjust aspects
in their view of their self-image.
Question #3: How do you feel this session on self-image will reflect on depression?
Desired Client Response: The client realizing where their self-image has negative aspects will allow them to make necessary changes that are
associated with eliminating their negative thoughts and start feeling better about themselves overall.
Description of additional supporting activities for individual to perform on their own: The client will periodically reflect on their self-image by
doing regular assessments in their journal using the outline of todays assessment.

1. As I reflect on the activity, to what extent will / were clients productively engaged? (Brief description of how they were or were not
productively engaged.)
The discussion of self-image kept the client engaged well because there was continual feedback from everyone in the group. Each client
discussed their self-image assessment and other clients gave them helpful feedback.
2. How and when will I know if the clients learn what I intended them to learn? (list objective)

As the clients reflect back on their self-image assessment and periodically reevaluate in their journal, their knowledge of the content will be
displayed by whether their self-image improves or stays the same/declines.
