She takes pictures are information through out the week and then works with a peer mentor every Tuesday to create her blog. B. Provide a detailed describe the barrier(s) that led to your design and creating this adaptation. Currently the student needs a lot of assistance from the peer mentor in order to complete her blog, she also has struggled with expressive language and responding using a widened vocabulary. She is also working to increase her topics of conversation. C. Describe the adaptation and how it will eliminate the barrier. The adaptation is change in strategy for blog and vocabulary completion and the utilization of new tools on the Ipad and explanation of current apps already in use. Write an observable/measurable goal for the student that integrates the use of their adaptation within the activity. The student will use her ipad to identify and take a picture of a novel item 2x a week on campus using the Cam Find app and then enter the items into the created template on Book Creator during weekly blog time. E. Describe the specific contributions made by team members.
If you did not physically meet or talk to a specific team member then you need to describe how this team member could be involved. The team members contributions might include such things as prompts, positioning of the student, positioning of the adaptation, specific use of the adaptation, any additional equipment or adaptations needed, amount
of response time given, construction recommendations and instruction.
Include as many team members as possible from the list below. Peers: peer mentors will help direct the student through the new process and assist with any steps that are difficult. Peers can also model using the new app and system. Occupational Therapist: The OT can work with the student on dexterity and typing skills with the Ipad and new apps Physical Therapist: The PT can assist with carrying, holding up, and managing the Ipad while walking and exploring campus Speech Language Pathologist: Speech can utilize the new template to facilitate conversations with the student about new topics and to expand conversation skills Parent: The parents can help the student practice the steps for the blog and using the app as well read and review the posts and talk with the student about the new items Teacher (special ed. and/or general ed.):Teachers can help by reminding the student to complete the assignment and then facilitating conversation based on the novel items Adaptive PE: N/A Other: F.
