I am a credit card holder with a card number ending with 3009. I have an outstanding obligation to your
company in an amount that is beyond my capacity to pay due to an ongoing crisis that my family
experiences. My father was confined in the hospital for more than a month and has undergone several
operations due to diabetes complications that has led to kidney malfunction and is currently undergoing a
weekly dialysis.
I would like to request that my remaining obligations to your company be approved for restructuring so
that I may be able to pay in accordance with my current capacity. Attached is the Summary of my
Monthly Expenses and my Certificate of Net Take Home Pay together with my father's medical
certificate.
