AUXILIARY FAILURE EVENT ANALYSIS REPORT

Auxiliary Failed : PA Fan 4 B tripped at 11:25 Hrs.

Brief description of Event : PA Fan 4 B tripped while LOP changeover LOP-1 to

LOP-2. C/M 4C also tripped (C/M 4A & 4B were U/P)

Observation : 1.Set not tripped.

2. C/M-4A & 4B were U/P
Analysis of failure : LOP no-2 Started. At the time of tripping of LOP no-1, PA Fan
4B got tripped. It is observed LOP no-2 is in trip condition After checking by TIC (vide
P/No.2801) it is observed that trolley contacts of LOP no-2 were loose. Same is
adjusted & trial taken found ok.
Action: LOP no.2-start command given from PCR but it was not started & LOPS
no.1hand tripped. Therefore PA fan 4B got tripped.LOP no.2 ckt checked trial given
to PCR
Root cause :

Recommendation : While changeover of auxiliary starting of auxiliary should be

confirmed from local before switching off running auxiliary
