As someone who knows far too much about nuclear war, I feel as though I can answer that question.

First lets
start with the weaponry, 1 modern nuclear missile has a minimum of 7 independent warheads, each warhead is
20 times more powerful than the bombs dropped on Japan...and some countries have THOUSANDS of missiles.
When a nuclear device explodes, it shoots a cloud of radioactive debris into the upper atmosphere, coating the
entire planet. Now imagine thousands of explosions, it would create a new climate on earth in a matter of
minutes, the entire earth. While countries that didn't have bombs dropped on them, would not have buildings
blown apart, they would suffer the same mass extinction of both plants and animals, and be covered in just as
much radioactive material as a country that was attacked.
In short, no matter where you are on the planet, the only difference would be if the buildings were intact or not.
Third world countries probably had even less survivors because there are few resources that would be protected
from radiation. Those that did survive would be killed by consuming radioactive food and water, those that
survived that would simply starve because there are really no stockpiles of supplies. So while the buildings are
standing, the population would probably be completely obliterated, if not by radiation, then by the swift and
dramatic climate change
