THE 2" STEEL MAIN WAS ISOLATED AT 10:58AM USING STOPPLE EQUIPMENT. THE MAIN WAS EXPOSED AND

HOME WHICH WAS LOCATED APPROXIMATELY 15 FEET FROM THE GAS MAIN. THE HOMEOWNERS HAD RECEN

IN THE RELEASE OF PRODUCT. THERE WAS EVIDENCE THAT THE EXCAVATOR FAILED TO VERIFY THE LOCATI

UNKNOWN SERVICE LINE AT APPROXIMATELY 11 INCHES OF DEPTH. INSTALLER EVACUATED PERSONNEL AND
