PURSUANT TO THE STIPULATION OF THE PARTIES HEREIN, THE COURT
ORDERS, ADJUDGES AND DECREES AS FOLLOWS:
1.

The Court finds that both petitioner and respondent have moved from the County of

NAME OF COUNTY since the order or judgment entered on DATE OF ORDER OR

JUDGMENT.
2.

The Court further finds that the petitioner is currently residing in the County of

NAME OF COUNTY and has been residing in said County since DATE OF MOVE and that the
respondent is currently residing in the County of NAME OF COUNTY and has been residing in said
County since DATE OF MOVE.

The Court further finds that the ends of justice and the convenience of the parties

would be promoted by a change in venue from the County of NAME OF COUNTY to the county of

NAME OF COUNTY.

That the venue for the above-mentioned case shall be transferred to the Superior Court

of California, County of NAME OF COUNTY, and that the parties shall pay all costs associated

with said change of venue and the Clerk of this Court is ordered to transmit all documents needed to

Clerk of the Superior Court of California, County of NAME OF COUNTY to effectuate said change

of venue forthwith upon the filing of this stipulation and order.
