Information from the Self-Assessment tool is used to develop SMART* goals related to student learning and professional practice. An action plan,
along with evidence to be collected, is developed to share with the evaluator. The educator and evaluator collaborate to refine or revise the goals
and plan as needed.

Students will improve metacognitive development in order to effectively self-monitor and hold themselves accountable
for their own learning.
This will result in fewer late and missing assignments, thus decreasing the number of students with Ds and Fs by 50%.
Current grades will be posted on a weekly basis, parents will be contacted as necessary, and students will attend weekly
tutoring and make-up sessions as necessary.
In order to analyze and monitor student progress and growth.
From October 2015 to June 2016.

Students will improve metacognitive development resulting in fewer late/missing assignments and a 50% decrease in the
number of Ds and Fs earned which will be accomplished by posting grades on a weekly basis, contacting parents of students
who are at-risk of failing, and providing students with tutoring and make-up sessions in order to enable students to effectively
self-monitor and hold themselves accountable for their own learning during the time frame of October 2015 to June 2016.

Standard 4 Students engage in metacognitive activity to increase understanding of and responsibility for their own learning.
Educator Plan Action Steps:
Teacher will update grades in a timely manner and current grades will be posted in the classroom on a weekly basis. Teacher will
send home progress reports for students who are at-risk of failing. Parents must sign progress reports to ensure that they are
aware of their childs grades. Parents may be contacted through email and/or phone to be informed about their childs progress.
Tutoring and make-up sessions may be scheduled each week to allow students to catch up and remain on-track. Student grades
will be analyzed and monitored.
Evidence:
Students will show growth in assessment scores and overall grade percentage.
There will be an increase in students turning in assignments on time and making up assignments accordingly.
There will be an increase in students displaying initiative by retaking tests and turning in assignments for higher scores.

Indicator 1 The teacher regularly facilitates two-way communication with parents and guardians, using available tools that are
responsive to their language needs, and includes parent/guardian requests and insights about the goals of instruction and student
progress.
Educator Plan Action Steps:
Parents will be contacted as necessary through Remind notifications, emails, phone, and letters sent home with students. Parents
will be informed of both exemplary and unacceptable behavior and/or grades. Students who are at-risk of failing will be sent
home with a progress report that must be signed by a parent/guardian.
Evidence:
Remind database will document all notifications and reminders.
Notes, letters, and other parent-teacher contact will be documented.
Student assessment scores and overall grades will be analyzed and monitored to determine the percentage of students
who have improved academically and behaviorally as a result of parent communication.
