I, JOSEPHINE AQUINO, Filipino, of legal age, married and a
resident of Brgy. Anibong, Infanta, Quezon, Philippines, after being
sworn in accordance with law, hereby depose and state:
1. That I am the sole heir of JOSEFINA CORONACION who died
intestate on _____________ at Brgy. Anibong, Infanta Quezon;

That said deceased left a certain parcel of land covered by
Transfer Certificate of Title No. P-4065 and more particularly
described as follows:
A parcel of land (Lot No. 3649, Case 4, Pls-1052-D), beginning
at a point marked 1 of Lot 3649, Case 4, Pls-1052-D, being S. 07-37
W., 3104.35 m. from B.L.L.M. No. 1, Pls-1052-D, thence S. 13-00 W.,
20.26 m. to point 2; 79-10 W., 38.84 m. to point 3; 13-26 W., 39-70 m.
to point 4; N. 84-14 W., 19.59 m. to point 5; N. 13-33 E., 30-72 m to
point 6; N. 76-50 W., 10.63 m. to point 7; N. 16-04 E., 73-32 m. to
point 8; N. 78-13 E., 29.04 m. to point 9; N. 18-01 W., 61.28 m. to
point 10; N. 75-10 E., 68.34 m. to point 11; S. 59-03 E., 25.72 m. to
point 12; S. 08-25 W., 59.65 m to point 13; S. 28-43 W., 71.97 m. to
point 1, point of beginning. Containing an area of ELEVEN
THOUSAND FOUR HUNDRED AND SEVENTY THREE (11,473)
SQUARE METERS. All points are marked on the ground as follows:
points 9, 10 & 11 by Stakes; and the rest by P.S. cyl. conc. mons.
Bounded on the E., along line 1-2 by Lot 3647, Pls-1052-D; on the SE.,
along lines 2-3-4 by Lot 3648, Pls-1052-D; on the S., along line 4-5 by
Provincial Road; on the W., along lines 5-6-7 by Lot 3650, Pls-1052-D;
along line 7-8 by Lot 3651, Pls-1052-D; and on the NW., and N., along
lines 8-9-10-11-12 by Bacong River; and on the E., along lines 12-13-1
by Lot 3646, Pls-1052-D. Bearings True. This lot was surveyed in
accordance with law and existing regulations promulagated
thereunder, by Mercilino Blanco, Geodetic Engineer, on July 18,
Septemeber 19, 1973; March 26, April 2, 1974 and approved on April
11, 1975. NOTE: This lot is covered by F.P.A. No. IV-3-12168.
3. That said deceased left no debts, nor any Last Will and Testament;
4. That pursuant to Sec. 1 of Rule 74 of the Revised Rules of Court
of the Philippines, I hereby adjudicate unto myself the abovedescribed parcel of land, and the improvements found and standing
thereon, and I hereby request the Register of Deeds to issue a new
Transfer Certificate of Title in my name;
5. That the above-described parcel of land does not come under the
operation of the Comprehensive Agrarian Reform Program.
