1. Specific incident you witnessed or the child has told you: His father punched him in the chest and shoved him against a
wall.

4. Pattern of neglect or abuse: He comes to school wearing cargo pants and a long sleeve shirt despite the hot weather. His
head was shaved. He has not been completing his homework or participating in class. He also seemed to be crying, but
says he cannot talk about it.

6. Any explanation provided by the child (in the childs own words): Tenacious says his father beat him because he did not
like his haircut. He also says that talking about the beating was a violation of a pledge, which includes not talking to

anyone without his fathers permission, obeying when told to do something, and getting a punishment if he does not not
follow these rules.

noises from Tenacious house.

menstruating.
9. Location: Her body in general has signs of abuse.
10. Severity of injuries: She often is dirty, hungry, and in need of feminine products.

11. Pattern of neglect or abuse: Joy has had head lice and on multiple occasions comes to school dirty, hungry, and in need
of feminine products. People have noticed her sitting by herself at lunch and at other school activities lately. Her mom
has not shown up for conferences.

13. Any explanation provided by the child (in the childs own words): Joy went to the nurse as she appeared lethargic. Joy
tells the nurse that she does not want to go home because her sister will be mad. Her sister said that she needed a pad,
but they did not have any. She did not tell her mom this because she came home happy from a party. Joy also shares that
she sometimes stays home wither her five nieces and nephews.
