her children.

STRENGTHS AND LIMITATIONS

Strengths: The client is extremely motivated and determined to find a job, get her
own place and get her children back.
Limitations: The client does not have much of a working background because she
has been a stay-at-home mother for a while. She has been a CNA and is working on
getting her CNA license back.

ASSESSMENT GOALS // SPECIFIC INTERVENTION

Assessment Goals // Specific Intervention

Goals for Tillie are to fill out resumes for all possible job openings, fill out housing
applications, and to help Tillie the best way in completing these tasks. Tillie has
somewhat of a low self-esteem, so motivational interviewing with her would be the
most fitting.

- I did not have any biases going into the interview with the client, I prepared myself to be open to
anything that was said from the client.

- I noticed that the client really loves and cares for her children and like any other caring
she wants the best for her children.

3.

What events or life experiences have shaped them into the person they are?

What is the staff recommendation about diagnosis, Interventions or treatment?

-New Horizons cannot make any diagnosis, interventions, or treatment; however, there
group meetings every Thursday that serves as motivational interviewing and support.

5. What changes do recommend for the agency or service (Needs of agency, changes, and
issues)?

mother,

-Growing up, the client was abused by her step-father and sadly was abused by her
husband;
however, this did not make her any less of a person. She is determined not to let this bring her down and
is fighting not only for herself, but for her two children.

4.

What were some value differences between you and the client?
