We are the solicitors for Christina Ryerse and, effective February 19, 2016, our client refinanced
the above property by placing a new first mortgage in favour of:

Kindly amend the above fire insurance policy to note the interest of the aforementioned first
mortgagee under the loss payable section and then forward a certified copy of the policy to the
mortgagee at the address shown above. It would be appreciated if you would also forward
confirmation of such change to our office.
We are enclosing an authorization signed on or behalf of the insured in this regard.

I have carefully examined the survey prepared by John Draftsman, O.L.S. and dated
September 19, 1982, a copy of which is attached hereto, and have compared my
knowledge of the buildings situate on the subject property with those shown on the said
survey.

The survey accurately describes the property and buildings on the subject property as they
exist today and there are no other buildings, additions, decks, swimming pools,
outbuildings or any other structures not shown on such survey, except as referred to

I am the registered owner of the above-mentioned property and have knowledge of the
matters hereinafter deposed to.

There is not currently and nor has there been within the past 45 days, any construction,
alterations, renovations improvements or building materials supplied to the subject
property.

The proceeds of this mortgage will not be used to finance any construction, alterations,
renovations or improvements to the subject property or to repay a mortgage which was
taken out for this purpose.
