CPSS Kristina Kall SYNOPSIS On October 23, 2015, Child Protection Investigator Danielle McCoy was assigned a Hotline complaint of physical abuse on a 12 year old male named a» ‘Same: (#2015-255381). It was alleged he had been punched in the face, and had his head slammed into a mirror by his mother, Keishanna Thomas. During this investigation, it was discovered ww sister, Janiya Thomas, could not be located when the children were being removed from the mother’s custody. C.P.I. McCoy documents her attempts to gather information from the mother as to Janiya’s whereabouts, however closed the case on October 9, 2015 without locating her. Furthermore, C.P.I. McCoy did not report her as missing to Bradenton Police Department, nor did she enter her as missing into the Florida Safe Families Network system (F.S.F.N.). On October 12, 2015, Child Protection Services Supervisor Kristina Kall approved the closure of the aforementioned case. Kall was responsible for reviewing the investigation to determine all appropriate action had been taken. She failed to ensure Janiya Thomas was entered as a household member, reported as missing to theMANATEE COUNTY 1A, #2015-10059 SHERIFF'S OFFICE CPi Danielle McCoy PROFESSIONAL STANDARDS CPSS Kristina Kall appropriate law enforcement agency of jurisdiction, and entered into the F.S.F.N. system as missing. C.P.S.S. Kall failed to notify supervision that a child in this report was missing. It was not until an email was sent to Director Lancsarics and Deputy Director Wilson on October 14, 2015 at 11:15 a.m. by Supervisor Shavonne Simon with the Sarasota Y.M.C.A./Safe Children Coalition, that supervision above CP.S.S. Kall was notified, INVESTIGATION On November 21, 2015, I was assigned to this case, received the case file, and reviewed the Allegation of Employee Misconduct Forms, (EXHIBIT “A”). I looked up and made part of the case file, a copy of General Order 1013, 12.3.1, (EXHIBIT “B”). Danielle McCoy, and approved by C.P.S.S. Kristina Call on October 12, 2015, (EXHIBIT “F”), On December 21, 2015 I had C.P.I. Danielle McCoy report to my office within Professional Standards, At that time I provided her the Admonition form and the Order of Confidentiality form. She reviewed and acknowledged the receipt of both by affixing her signature. At that point I conducted an audio recorded, sworn statement of C.P.I. McCoy.
