Practice Prescription Writing Sheet


Do the following five prescriptions the best that you can. We will go over them in
class on January 23rd.
You will need to look up how some of them come from the pharmacy (childrens
medications) in order to correctly write the prescription. Think about what you are
writing and make sure to fill out everything. This is not for a grade it is to
practice.
1. Your patient, Nancy Drew, born on July 4, 1990. Has had her yearly exam
that was normal and she wants to go on birth control pills. You will order for
her Sprintec Birth Control Pills. Write the prescription.

Your patient, Frank Enstein, born 9/7/ 1960, needs a refill of his blood
pressure medications. He takes Lisinopril 20 mg daily and is well controlled.

3. Bobble Head, born 8/23/1970, needs his Lasix refilled. He takes 20 mg daily.

4. Hebe Smart, born 3/8/2011, has an awful ear infection. He weighs 36


pounds. You are going to give him Amoxicillin 90 mg/kg/day in divided doses
for 10 days.

5. Sylvester Puttycat, born 11/04/2013, weighs 10 pounds 11 ounces. He needs


Cefdinir at 14mg/kg/day for 5 days in divided doses.
