Reason for Referral: B was referred by his teacher due to concerns about his
fluency.
Parent interview: Mrs. B does have concerns about Bs fluency. She reports that
he repeats endings while talking and reading-- for example, going-ing. She
states that "it's gotten better over the last few weeks but it had been worse". She
reported that there is no family history of stuttering.
Teacher interview: Mrs. K reported that B exhibits stuttering through repetition of
end sounds/ ending words in a sentence.
Interventions documented by the classroom teacher(s) and others who work with
the student: Mrs. K has not been successful with any interventions in the
classroom.
Educational impact of the communication pattern on the students performance in
the classroom: Mrs. K reported that his fluency pattern is interfering with the
students learning in the areas of reading, specifically when he's reading out loud.
Informal measure(s): All structures were functional for speech production and
developmental age. Current dentition is intact, and he presents with no
abnormalities in his oral structure.
Classroom Observation: B was observed in class on May 20, 2015, during a class
reading activity. For his fluency, B presented part word repetitions and
prolongations. Part word repetitions were heard as he was reading such as "this"
in "This is a great place" and "are" in "The flowers are so colorful". Prolongations
were observed when he was reading such as "are so" in "The flowers are so
colorful", "the San" in "Today, we're in the San Gabriel Mountains", and "one" in
"Last one in the car is a rotten egg". It was noted that B was fluent when reading
in unison with his classmates. In regards to his pragmatics, B appropriately takes

turns and has appropriate eye contact. He exhibited the ability to follow 1 step
directions, to begin the task appropriately, and remained focused on the task.
Standardized test administered:
Fluency Testing: B was given the Stuttering Severity Instrument, SSI-4. The SSI-4
assesses stuttering severity as measured by the frequency of stuttering
moments, duration of the longest stuttering moment and any physical
concomitants that accompany stuttering. Several spontaneous speaking samples
were analyzed to obtain this information. During the spontaneous speech tasks,
B stuttered an average of 6% of the time. During the reading task, B stuttered an
average of 6% of the time. He demonstrated the use of part-word and whole word
repetitions as well as prolongations in both his speech and reading. The average
duration of stuttering moments was a half second. He did have physical
concomitants that were observed. He was observed to produce noisy breathing
that was barely noticeable to a casual observer as well as facial grimaces like jaw
jerking, lip pressing, and jaw muscles tensing but these behaviors were not
noticeable unless looking for it. He obtained a Total Score of 19, with a Percentile
Rank that was between 24-40. That placed his performance in the Mild severity
rating.
Summary: B is a 8 year-5 month old second grade student whose teacher referred
for the evaluation due to concerns that she had about the impact his stuttering
was having on his academic performance. She was specifically concerned about
how his stuttering was impacting his oral reading. He was observed, and tested
using the Stuttering Severity Instrument, SSI-4. He demonstrated whole word,
prolongations, and part-word repetitions during spontaneous speech tasks and
during reading. He demonstrated the part-word repetitions and prolongations in
reading during the classroom observation. He was found eligible for services
under the requirements for classification for the presence of a fluency disorder in
NJAC 6A: 14-3.6. You will be receiving a meeting notice to discuss these results
in person. It was a pleasure to evaluate your son.
