Goals
1. Goal 1: By April 4, 2016 I will demonstrate a body of knowledge from nursing and other
disciplines concerning current and emerging health care issues by engaging in effective
and efficient therapeutic communication with vulnerable elders who have been diagnosed
with dementia in a long-term care setting (College of Nurses of Ontario, 2014).
2. Goal 2: By April 4, 2016 I will demonstrate a body of knowledge in nursing science by
performing accurate blood pressure measurements on three (3) residents in a long-term
care setting (College of Nurses of Ontario, 2014).
3. Goal 3: By April 4, 2016 I will demonstrate my knowledge of population health; more
specifically the health of geriatric residents in a long term care setting. I will be able to
distinguish determinants of health that are unique to each individual resident I work with,
thus allowing me to be able to provide client-centered support for the activities of daily
living (College of Nurses of Ontario, 2014).

Activities, Necessary Resources and Timeframes to achieve my goal


Goal 1:
I will review and understand my therapeutic relationship/communication notes from
NURS1000 (Individual as a Nurse) by no later than January 18, 2016.
I will recall the labs from NURS1000 (last semester) where I learned proper
communication skills, and continue to implement these skills in all labs and clinical
placement over the course of this semester and throughout my degree.
I will refer to and study the Therapeutic Nurse-Client Relationship practice standard
document provided by the College of Nurses of Ontario (CNO) by February 19, 2016
(College of Nurses of Ontario, 2006).
I will develop a better understanding of dementia by using reliable sources such as the
Alzheimer Society of Canada website by February 15, 2016 (Alzheimer Society of
Canada, 2016).

I will look to nursing specific and elderly specific documents such as Nursing Best
Practice Guideline: Screening for Delirium, Dementia and Depression in Older Adults
and Nursing Best Practice Guideline: Caregiving Strategies for Older Adults with
Delirium, Dementia and Depression by February 21, 2016 (Registered Nurses
Association of Ontario, 2003; Registered Nurses Association of Ontario, 2004).
I will consult with my professors, clinical instructor, lab instructor, peers and personal
support workers (PSWs) over the course of the semester about any inquiries I may have.
I will ensure that I study and use all the resources I can (such as those listed above). I will
then apply the skills and knowledge to the best of my ability when communicating with
residents who suffer from dementia over the course of this clinical placement.
Goal 2:
I will re-watch the provided vital signs and cardiac assessment Panopto videos that are
posted on my Blackboard Learning System by January 25, 2016.
I will use Nursing Skills Online 3.0 as a reference for vital signs and cardiac assessment
skills by January 25, 2016 (Elsevier, 2016).
I will practice blood pressure measurements on my family and friends on a regular basis,
as I own my own blood pressure kit.
I will consult my clinical instructor and lab instructor when in need of assistance or
clarification. Furthermore, I will watch others and take note of proper technique. I will do
both over the course of the semester.
I will use the Cumulative Index to Nursing and Allied Health Literature (CINAHL)
database to conduct a literature search about the variations in blood pressure, factors that
can affect blood pressure and the normal blood pressure range for different age groups by
February 7, 2016.
I will identify normal blood pressure ranges for different populations so I am prepared
and can act accordingly. In addition to this, I will also check health histories of residents
so I know what is normal for each of them.
Goal 3:
I will use the Cumulative Index to Nursing and Allied Health Literature (CINAHL)
database to conduct a literature search on population health; more specifically
determinants of health in geriatric individuals by February 15, 2016.
Over the course of the semester I will acquire feedback from the residents and my clinical
instructor about my approach to client-centered care.
Over the course of the semester I will refer to my NURS1002 (Introduction to
Foundational Practice Concepts) notes as a reference about geriatric care.
I will read the Registered Nurses Association of Ontarios (RNAO) Clinical Best
Practice Guidelines: Person-and-Family-Centered Care by January 25, 2016 (Registered
Nurses Association of Ontario, 2015).
Before each clinical placement I will identify situations where client or family beliefs and
wishes could hinder the appropriate delivery of client-centered care.
For each resident that I am assigned, I will conduct thorough research about their medical
history that could impact their activities of daily living.

Evaluation
(How will I know that I have achieved my goal?)
There are various ways in which I will know that I have achieved my goals. These include:
I will communicate and ask for feedback from my clinical instructor, PSWs, nurses,
residents and peers. I will continuously ask for this feedback over the course of my
placement to ensure that I can use their advice to either change or maintain my actions.
Establishing rapport with these individuals will ensure that I have developed trusting
relationships. By asking for feedback throughout my placement, I will be able to adapt
my knowledge and skills and accomplish all my goals.
Establishing confidence with my communication skills, my understanding of dementia
and expanding on my knowledge about vulnerable populations (e.g. elders, individuals
with dementia), and then correctly applying this knowledge to the action of therapeutic
communication will ensure that I have accomplished my first long term goal. Effectively
and efficiently communicating with at least one resident with dementia, and having this
result in a positive and therapeutic way will ensure that I have reached this goal.
I will know that I have achieved my second long term goal when I have established
confidence in the skill of blood pressure measurement and once I have taken three
accurate blood pressure measurements. Furthermore, being able to complete these three
measurements in with proper technique and in a timely matter will further improve my
confidence and solidify the accomplishment of my goal.
By developing an understanding of determinants of health and feeling confident in those
that are specific to geriatric individuals will ensure that I have accomplished my third
goal. In addition to feeling confident, receiving feedback from residents and seeing
residents excel in daily activities of living due to my support will ensure that I have
accomplished my goal of providing individual client-centered care.
Accomplishment of these goals will allow me to feel confident in applying these skills in
my future practice.

My learning plan addresses:


Self Reflection
I have chosen my three goals based on personal reflection and concrete events that have occurred
in my life. My goals are based on interest, the desire to acquire new knowledge and skills and the
desire to advance my current knowledge and skills. My intentions are that these goals will
provide me with guidance throughout this clinical placement, as well as in future clinical
practice. Further self reflection about each specific goal can be found below.
Goal 1: I have a family member who suffers from dementia and over the past few years
her condition has worsened. Watching others (e.g. my family members) unsuccessfully
communicate with her has encouraged me to develop a further understanding of the
disease, as well as learn therapeutic techniques and effective ways to communicate with
individuals who have been diagnosed with dementia.

Goal 2: Over the course of this semester we have learned various new skills in the lab,
including taking blood pressure measurements. We were introduced to this skill in our
third week, and since this time I have struggled to master this skill. Understanding how to
accurately measure blood pressure is a very important skill for a nurse to have, as blood
pressure can be an indicator of both positive and negative health. Therefore, due to the
fact that I need to improve this skill I felt it was important for me to incorporate it into
my goals.
Goal 3: Independent individuals have the ability to support themselves in their daily acts
of living. One of the main reasons I chose the nursing profession is because I want to help
dependent individuals and make a difference in their lives; my third goal incorporates this
concept. Being able to distinguish individual determinants of health will allow me to
provide the best care specific to each resident. Understanding how to provide clientcentered care will allow me to properly assist individuals, resulting in positive outcomes.

Consideration to previous evaluation
Not applicable as this is my first placement and I have had no previous evaluations.
Cumulative and Appropriate for education level
This learning plan is both cumulative and appropriate for my education level. I have designed my
learning plan so it will allow me to incorporate the skills I have learned and will learn from my
courses, labs and the literature. Furthermore, my goals coincide with the course objectives.
