respiration status suddenly changes then I monitor their oxygen saturation levels to decide on whether they are in need of better
positioning, deep breathing excersices or administration of oxygen via nasal prongs.

aspects of a clients diagnosis that I need to focus more on. Example, a patient with a previous stroke my be at risk for another stroke
occurrence so I would continuously monitor vital signs and neurological status, including level of consciousness, motor and sensory
function, pupil size and reactivity, SaO2, and cardiac rhythm.

particular in how they wanted their feet to be moved so I sought help from a nurse who already had experience moving this patient. We
both were able to position the patient according to their liking.

pay attention to the persons tone of voice. For example, I had a patient who said they were cold and so I raised up the blankets to cover
them, but then they said no my head. So I repeated what the said just to clarify what the issue was. Their head was cold. So I asked if they
had a hat and then the patient said no and cussed their teeth so I tried to figure out what I could use quick. I noticed a scarf around their
neck and asked the patient if they wanted my to wrap the scarf around their head and then they became noticeably happy.

unfamiliar approaches. For example, working with patients who are confused and refuse to take their medications. Other nurses find ways
of talking to them in simple language to inform them that the medication is necessary. They are quick and don't allow time for the patient to
refuse.

deep vein thrombosis when they didn't have that anyways. I explained that the lack of activity in bed can increase the risk for a clot to form
and travel to the heart and this medication will prevent those clots from forming and becoming a issue.

are on restraints. For example, I was asked to feed a patient who was on a thickened fluid diet and they were on wrist restraints. The patient
refused to eat and requested to have water. I was not able to give them water and I noticed that they did not look at me when I was talking
to them so I told them I will be back. Because of their body language I knew I wasn't going to get very far at that time so I waited five
minutes and went back and said I will open this vanilla flavored drink so that you can try some. At first they said no but when I put the
straw closer to their mouth they begin to drink.

had a patient who requested me to warm up their heating bag and the nurse thought it would be best if I waited a couple minutes before
giving it to the patient because it was too warm for the skin. I agreed with the nurses perspective and we both thought it would be best if
the heating bag would be wrapped in a towel to decrease the chances for skin burn.

patient who had a bed by the window and even though the curtain was closed all the way there was still chance for someone to look
through if they were close to the window. There was a gentleman visiting another patient who decided to look out the window by my clients
bed. This made my client feel unconfortable and when I noticed the gentlemen I quickly covered my patient with the blankets and poked my
head out of the curtain to ask him to kindly move away from the area until I was finished. He didn't even realize what was going on until I
said it but he happily complied and apologized. My patient was more relaxed afterwards.

V.R. : I support clients rights for self-determination and choice whenever the food tray comes. The nurse usually informs me that the
patient needs everything opened on their food tray. Sometimes I have patient who like to try to open the utensil package on their own.
When I notice they are struggling I ask if they would like some help, when they say no then I respect their wishes and stay close by incase
they still need my assisstance.

was helping a colleague move a patient up in bed however while we were sliding the patient up the patients head dragged and so I
requested to stop and lower the head of he bed some more so that we could avoid dragging the head.

V.R. : I make sure it is safe to move patients before doing so. For example, a porter was waiting for the nurse of a patient who came back
from their tests and they had asked if I could assist them in the transfer. I told them I will make sure with the nursing staff if they will allow
it. I went to look for the patients nurse, who was busy documenting, and asked if it was okay to move the patient using just two personnel.
She said it would be fine. So the porter talked me through transferring the patient from one bed to the other.

before and after helping my patient on and off the commode chair I wipe it down using a cavi wipe and I wait for it to dry. I also organize

example, I had a patient who was experiencing pain and there pulse and respiration had gone up because of their anxiety caused by the
pain. I assessed their pain level and offered pain medication to reduce the pain. I also made sure to leave them in a comfortable position.

knowledge and skill by carefully handling medications. For example, I had a patient who needed to take medication that was toxic to touch
and could cause cancer if it was in contact with skin. I put my gloves on and made sure to place the pill inside the patient's mouth using
gloves so that the patient had no contact with the toxic drug.

example, I had a patient who seemed drowsy the entire morning. The patient had no interest in food but rather wanted to rest. I brought it to
the nurses attention. The nurse confirmed that this was not his patients every day status and that the drowsiness was linked to the type of
medication given. The nurse went to go talk to the physician about possibly decreasing the dose.

changing a patient who is in incontinent during a bed bath. By changing my patients brief at the same time as giving a bed bath I can
reduce the amount of turning and re-positioning that they would have to do. While the patient is exposed I can do my head to toe
assessment.

achieve mutually agreed expected outcomes whenever a patient needs to mobilize. For example, I was caring for a patient who was not
interested in eating. The nurse said that this patient needs to eat and that getting them up in the chair may motivate them to eat instead of
sleeping through mealtime. It took a lot of encouragement to get the patient up, but they were content after sitting in the chair. They ate
very little, but it was better than refusing it entirely.

that they would not pull out their IV lock. When I assessed the patient in the morning they were not confused and the nurse told me it was
okay to remove the restraint. The patient became noticeably happy after the restraint was removed.

patient to do some exercises. For example, after giving the patient pain medication I instruct them that I will come back in about 15 minutes
to assess their pain level, if they feel like the pain is mild than before than I ask them to move their feet in a plantar and dorsiflexion motion.
This helps reduce deep vein thrombosis in bed ridden patients. The client is more likely to comply to more exercise moments when they
are pain free.

V.R. : I had a patient who requested to be put on oxygen and so I got my clinical teacher to help assist me in telling the patient that they

receive more oxygen via nasal prongs then that could harm the patient's health.
