SOAP Note
S: Dylan entered the speech therapy room and was happy and motivated to start the session. At
times during the session, Dylan started to engage in task avoidance, but the clinician was able to
redirect him and keep him engaged and motivated for the rest of the session.
O: Dylan is making good progress to meet and master his goals.
Goal 1: Dylan will follow directions with the preposition next to in a field of 1 over 3
consecutive sessions with 80% accuracy.
He was able to perform the directions with 75% accuracy with verbal prompts and intermittent
models.
Goal 2: Dylan will label pictures of vowel-consonant-vowels words for previously taught
productions for at least 10 pictures over 3 consecutive sessions with 80% accuracy.
He was able to label the 10 pictures with 60% accuracy with faded verbal prompts.
Goal 3: Dylan will perform 25 trials of oral motor exercises for lip closure using horn #4 with
audible sound given when providing jaw stability.
He performed the 25 trials and started to produce louder sound productions, and therefore met
the criteria for the goal.
Goal 4: Dylan will produce /f/ in initial position for phrase approximations given visual cue of
the clinicians face with 80% accuracy.
He was able to produce the /f/ initial phrase approximations with 87.5% accuracy, and therefore
met the criteria for the goal.
A: Dylan is making good progress with these goals. For the oral motor exercises, he was able to
perform the 25 trials and met the criteria for the goal, but required prompting to take a deep
breath before each trial. When he was prompted to make the sound louder so others across the
room could hear it, he was more motivated to produce a stronger sound production and was able
to do it more often than during past sessions. For the goal concerning following directions with
the preposition next to, the student needed verbal cues and his progress indicates that he is
learning this new preposition and that the goal is appropriate for him. For the goal concerning the
VCV words, he was able to complete this task with minimal prompting and his progress suggests
that this skill is emerging. For the goal of producing /f/ initial phrase approximations, he was
able to meet the criteria for this goal with minimal to moderate visual cue to look at the
clinicians face.
P: For the next session, the plan is to have Dylan continue to practice his oral motor exercises to
address his lip closure until he is able to produce loud productions more consistently. He will
continue working on following directions with the preposition next to in a field of 1 until he
masters that goal. Upon completion of that goal, the context will be changed to a field of 2. He
will also continue to work on the VCV word picture labeling and /f/ initial phrase
approximations until these goals are mastered.
