SOAP Note
S: Ali-Kahn appeared ready and motivated to work at the beginning of the
session, but did frequently place fingers into mouth. He demonstrated
adequate to good attention to tasks, participation, and cooperation.
However, at times he appeared excitable, distractible, and impulsive. He had
a playful, happy, and pleasant demeanor. At the end of the session, he
started to whine and had a little difficulty transitioning back to classroom,
but was easily calmed.
O: Ali-Kahn is making modest progress towards his goals.
Goal 1: Ali-Kahn will respond appropriately to yes/no questions when
participating in a structured speech activity, given verbal, visual, and tactile
prompts with 80% accuracy.
He was able to perform this task with 50% accuracy with frequent phonemic
cues to elicit no responses.
Goal 2: Ali-Kahn will produce spontaneous 1-3+ word utterances, word
approximations or signs, to express pleasure or displeasure, given verbal,
visual, and tactile prompts with 80% accuracy.
He was able to perform this task with 50% accuracy with occasional verbal
prompts.
Goal 3: Ali-Kahn will consistently comprehend and perform simplistic, 2-step
directives, given verbal, visual, and tactile prompts with 80% accuracy.
He was able to perform this task with 30% accuracy with occasional handover-hand and light physical assistance.
A: Ali-Kahn is making some progress with these goals. For the goal
concerning answering yes/no questions, he was able to answer the yes
questions. However, he frequently said yes to the no questions as well and
required phonemic cues to elicit the no responses. For the goal concerning
spontaneous phrase production, Ali-Kahn mainly produced imitative phrases,
jargon, and echolalia in the beginning of the session and was unintelligible at
times. As the session progressed, he presented with increased spontaneous
productions and spontaneously sang along with Days of the Week song. For
the goal regarding following 2-step directions, he needed frequent handover-hand assistance to complete tasks and perform the directions in the
presented activity. Additionally, he often required the 2-step directions to be
segmented, as he did fair with one-step directions and poor with 2-step
directions.

P: For the nest session, the plan is to continue to have Ali-Kahn working on
answering yes/no questions, and promoting more spontaneous no
productions. Also, he will continue to work on following 2-step directions with
increasing independence and decreasing the need for directions to be
segmented. Additionally, we will work on eliciting spontaneous 1-3+ word
utterances in different engaging activities in order to facilitate more
spontaneous and functional language.
