Lesson Plan Two Assessment- Me on the Map Activity

Each student will be assessed based on the completeness of his/her project using the rubric
shown below. The teacher will look for completion of each step of the directions (labeling
the flaps, having the correct number of flaps, having pictures drawn that correctly match
the label of each section, and correctly placing the maps on the bulletin board next to the
students photo). The adapted version below the original is for a student with complex
instructional needs. This student will be given a pre-constructed and pre-labeled Me on the
Map constructed by the teacher before the activity. This student will be graded for writing
his/her name neatly on the back of the project. This is a skill the student is working on in
his/her IEP and must be a recorded to assess progress. This student will also be graded for
drawing pictures under each section that correctly represent the given label (HOME,
SCHOOL, CITY and STATE), as well as for correctly placing his/her project in the correct
place on the bulletin board. The student will additionally be assessed by his/her ability to
correctly describe each of the sections listed above with at least 80% accuracy when
prompted by a teacher.
