Transition Planning Inventory Second Edition
Our purpose for using the transition planning inventory is to provide a
straightforward yet comprehensive way to identify the strengths and needs of our
students who are preparing to move from high school to any number of post-high
school settings.

Carver High School has 12 special education seniors in the 2015/2016 graduating class.
Six have acceptance letters from Lone Star College
One was accepted by Texas School for the Blind and Visually Impaired
7 out of 12 students are transitioning to post-secondary education.
I will continue to work with the five students who have not committed to any post-high school
settings.
