ANALYZING HAZARDS

Topic 5

Hazard Analysis
Analysis used in the workplace to

identify potential hazards with

objective to prevent accident

WHAT IS HAZARDS
A hazard is any source of potential damage, harm or

adverse health effects on something or someone under

certain conditions at work.
Basically, a hazard can cause harm or adverse effects (to

individuals as health effects or to organizations as

property or equipment losses).

1) Root Cause Analysis (RCA)

2) Change Analysis
An investigating method used in problem-

solving and determining the cause of

accident
Analysis based on comparing situation that
does not exibit a problem to one that does.

Step 5

Step 6

Step 7

Define Problem. What Happened?

Establish the norm What should have happened?
Identify, locate and decribe the changes what, where, when, to what extend?
Specify what and what was not effected

Identify the distintive features of the changes

List the possible causes

Select the most likely causes

Consideration
Single accurance
Focus on element of factors changes
Used of 5W (what, where, who, why and

when) and 1 H (how).
