Objective: to increase student awareness of his/her learning progress by independently

demonstrating critical thinking, problem-solving, and decision-making skills. The purpose of
self-evaluation is to enhance clinical skill development.
Assignment: the student will submit weekly written Self-Assessments by the end of each week
and address the following:
1. Describe what you feel you did well in clinic today/this week
This week was the first time I was able to clean all of the sub and supra off of patient in
one try! I also passed my nitrous formative and completed all of my required injections

2. Describe what did not go well in clinic

I could still use some more practice on not shaking while aspirating when giving an
injection.

3. How can I plan for improvement to meet these challenges the next time they present?
I will remember to keep my arm close to and tucked to my side and use a fulcrum. I will
also be conscious not to insert the needle deeper when I go to aspirate.

4. My clinical instrumentation and patient treatment goal(s) for the next clinic session is/are
This next week I will be completing my last patient over the course of two clinic
sessions! I will be using the caviton and my instruments are sharp!
