The dive culvert exhibits very moderate to heavy deterioration at the inlet and minor deterioration at the outlet
structures. There is a large accumulation of rock, branches and debris at the inlet.
Notes For Next Inspector:
This Dive Culvert is located west of the Hulberton Road Bridge between the bridge and the park.

Notes: Previously Rated 4 The inlet structure exhibits moderate to heavy deterioration. The worst conditions are
found at the east and west wingwalls where the top and free end surfaces are moderately to heavily spalled. The
spalling extends for approximately 2/3s of the wingwall length and reaches 6 deep. The concrete within the spalls
crumbles easily when struck with a hammer. The vertical surfaces on both wingwalls exhibits mapcracking affecting
100% of their surface areas with moderate efflorescence leakage. Additionally, just above the culvert opening, the
west wingwall has a horizontal crack with profuse leakage exuding near the junction with the headwall.
The headwall exhibits small areas of honeycombing and spalling. The majority of the headwalls vertical surface is
affect by mapcracking with moderate efflorescence.

Notes: Previously Rated 5 The trash rack is partially blocked by a large accumulation of rock, branches and debris.
The accumulation does not appear to impede the function of the dive culvert since there is no defined upstream
channel or ditch. The rating has been lowered to 4.

Notes: Previously Rated 4 The inlet structure has an area of seepage just above the top of the culvert opening on
its headwall and west wingwall. The leakage is profuse at the junction of the two walls. There is no relative distress
found in the embankment adjacent to the structure.
The embankment adjacent to the northwest corner of the structure was observed to be only slightly moist during this
inspection.
The rating will remain 4.
