1. Purpose
The purpose of this procedure is to explain the use of SC Safety Observation Card as the tool for risk
identification / suggestion
2. Scope
This procedure shall apply to all SCM managed business and vessel in fleet. The SC Safety Observation Card
can equally be utilized to commend someone for applying a safety practice or suggesting an idea to improve
QSHE . The SC Safety Observation Card is an observation and communication tool for use by all employees,
Client representative, Contractor staff and visitors. Its primary function is to bring to the attention of their
immediate Supervisors / Masters and Managers any potential or actual unsafe acts and conditions occurring at
their work site or on board vessel.
3. Responsibilities
Employees
To report any unsafe act, Safe act, Unsafe condition / practice, Suggestion etc using SC
Safety Observation Card as a tool to communicate to their supervisor
Site Supervisors
Ensure that this procedure is in place and that it is followed and the cards are closed in a
reasonable time frame.
Safety Officers
Ensure that adequate competence, training for the use of SC Safety Observation Card is
provided to all employees in his area and maintain register
QSHE Dept.
Ensure that the SC Safety Observation Card activity has undergone the proper level of
review and advises where additional action is warranted. Responsible for the
implementation and review of this procedure
4. SC Safety observation Card
4.1 General
The format of the SC Safety observation Card is designed to be pocket sized and be completed with
minimal effort. This SC Safety Observation Card shall be placed around work site or on board vessel in
conspicuous locations or made available on request to all employees, visitors and contractor staff.
4.2 Form completion
Part 1. Description of hazard identification / Safe practice / Suggestion
Name of person and their employing company originating SC Safety Observation Card
The exact location, date and time where the observation, opportunity for improvement or
suggestion observed shall be fully described
The actual observation, opportunity for improvement or suggestion shall be briefly described

Part 2. Corrective action / Suggestion


If the observer was able to correct the unsafe act or condition. The appropriate check box should
be marked
With the Safety Office / Observer shall describe fully any corrective action taken to provide a
recommendation, suggestion for improvement as appropriate
Part 3. Site Supervisor / Master Action & Comment
The responsible Site Manager/ Master shall describe the final corrective action taken. They will
also be responsible for determining the quality of the input and value added with regard to
suggestions for improvement. Ensure that the cards are closed in a reasonable time frame.
5. Record
The Safety Officer / Master of vessel shall keep records on the number of cards received over a month period
and report the number received in monthly QSHE statistics.
