Parts A & B

History
Bob Berry is a twenty nine year old married Caucasian male who lives
with, in wife in his mother in laws basement with his wife and their daughter
(age three). He is self employed as a mechanic but work is not very stable.
Mr. Berry is coming to see a social worker today because of the stress he is
experiencing because of his work and living situations, and the guilt he feels
because he has a son he has not seen in two years.
Family of Origin
At time of Mr. Berrys birth, his father was eighteen and his mother was
sixteen. His parents were not married when he was born. Mr. Berry was one
younger half-sister, Samantha. Samantha is 20 and is only the daughter of
Mr. Berrys mother. Samantha is a nurse aid at the local hospital. Samantha
is single and has no children. Mr. Berry was not a planned pregnancy for his
mother, but his sister was a planned pregnancy. Mr. Berrys parents were
never married they were young and broke up before Mr. Berry was born. Mr.
Berrys father passed away in a motor cycle accident at the age of 20. Mr.
Berrys mother is still living and is forty five years old. Mr. Berry describes
his home life growing up as happy. He celebrated Christian holidays such as
Christmas and Easter. His family would spend all holidays with his maternal
grandmother. Neither of His parents or himself graduated high school. His
sister Samantha graduated high school and technical school. His mother and
sister both appear to be in good health. In his family death was not talked

about much. He never really talked about his fathers death with his family.
There were no implications of abuse.
Developmental History
Mr. Berry was carried to term at 40 weeks. When he was born he was
seven pounds six ounces, and twenty inches long. He was born vaginally and
was bottle fed as a baby. He is unaware of any feeding issues or the age
that he was weaned. He walked around one year, he potty trained around
the age of two. Mr. Berry was never informed of any developmental delays.
Health History
At birth Mr. Berry was healthy. Throughout childhood, Mr. Berry had
chicken pox, strep throat, and lice. He never suffered any chronic illness, or
broke any bones. He did have stiches once on his chin when he was eight.
Mr. Berry has had no surgeries or other medical intervention, and is on no
medications.
Relationship / Sexual History
As a child Mr. Berry had many friends and was active in sports.
Currently he is not in contact with anyone from childhood, except his best
friend Ryan. Mr. Berry and Ryan have been friends for fifteen years. His first
girlfriend was at the age of fourteen. He is heterosexual, and had his first
sexual encounter at the age of sixteen. He enjoys sex, and has the ability to
reach orgasm. Currently, he is only sexually active with his wife. His wife has
had her tubes tied, and the two have no known stds.
Family of Creation History

Parts A & B

Mr. Berry was married when he was twenty four and his wife (Lillie) was
nineteen. The two were married because they were high school sweet
hearts. Neither actually graduated high school. Lillie works nights at a gas
station as a clerk. They share a daughter Violet who is three and was
planned. Mr. Berry states that he is happy in his marriage, and loves his
daughter very much. Mr. Berry also has a son Mikey who is eleven and was
unplanned. Mikey lives with his mother who Mr. Berry was never married to.
The two broke up while his ex was pregnant. There have been no other
pregnancies. Currently, Mr. Berry is involved in a relationship with his wife
Lillie, and daughter Violet. Mr Berry has not seen his son in two years.
Substance Use/Abuse History
Mr. Berry admits to having trouble with prescription pain pills in the
past. He began taking Vicodin at the age of thirteen. He used the drugs for
ten years and has been clean for six years. In the past he would take up to
thirty pills in one day. Mr. Berry has remained clean and active in NA.
Although he is currently clean, Mr. Berry does smoke cigarettes.
Mental Health History
Mr. Berry is currently not under any treatment for mental health. In the
past He was prescribed Xanax for anxiety. He took his prescription until he
decided to quit taking Vicodin, at which time he gave up Xanax as well. The
Xanax was helpful for him at the time, be he does not feel the need for
medication now.
Military History

Parts A & B

Mr. Berry has no military history.

Financial History
Growing up Mr. Berry lived in poverty. He was raised on government
assistance. Currently, his life isnt much better. He has irregular work as a
mechanic, while his wife is a clerk at a local gas station. The couple is
currently receiving food stamps.
Legal History
Mr. Berry has no legal history.
Immigration Issues
Mr. Berry was born and raised in Michigan and has no immigration
issues.
