Mrs. Gayatgay, G1P1, during her 3 month pregnancy, she went for ultrasound and found out
that her baby has hydrocephalus. She was positive for UTI and had pneumonia and no medication was
taken. She underwent caesarian section in Iloilo on May 2009, full term because the baby͛s head was
too big. She did not breast feed her child and on milk formula-Progress. Lemuel Gayatgay was diagnosed
with congenital hydrocephalus and after 19 days, he was placed with VP shunt. On June 2009, they went
to PCMC because of VP shunt malfunction where it can͛t drain anymore because of plugging. His VP
shunt was replaced and after a few days he developed infection. He stayed until October 2009 and was
discharged with take home anti-bacterial oral medications. On May 22, 2010, the parents noticed
erythema and leakage on the VP shunt site, hence Lemuel was admitted. His VP shunt was removed in
the ward because it was accidentally ͞hatak͟ on the father͛s shirt while the father was carrying Lemuel.

2 Co-Amoxiclav 450mg/5ml for 7 days- anti-bacterial
