Dear Sergio:

We are reviewing your financial aid application for the 2010-2011 academic year. Your file is currently
incomplete and processing cannot continue until you provide or update the following information:

- 2009 Student Federal Income Tax Return

Provide signed copies of your/your spouses 2009 Income Tax return (including schedules). You
must also submit proof of any other taxable/non-taxable income received in 2009. You may
request a tax transcript or Return to Taxpayer Form (RTFTP) by calling the IRS at 1-800-829-
1040. Form 8453 is NOT acceptable.

- 2010-2011 Independent Verification Worksheet

You must complete, sign and return an "Independent Verification Worksheet". You may access
this form on our website at www.mdc.edu/financial_aid/important_forms.asp.

- Proof of U.S. Citizenship Status

The Federal Processor has reported that the Social Security Administration was not able to verify
your US citizenship status. You must submit proof of your U.S. citizenship status, e.g. US
Passport, Naturalization Certificate, US Birth Certificate, etc.

- Selective Service Proof of Registration

Submit proof of Selective Service registration. If you have registered with Selective Service and
do not have proof, please contact the Selective Service Office at (847) 688-6888. If you need to
register, you can do so at: www.sss.gov.

Please submit the required documents to the address indicated above as soon as possible. A delay in
responding to this request may result in a loss of funding that might otherwise have been available to
you.
