USE: treatment

dermatophyte infection of fingernails or toenails in nonimmunocompromised


pt.

oropharyngeal esephageal candidiasis

ACTION: inhibits enziymes necessary for integrity of the fungal cell


membrane

ADVERSE AND SIDE EFFECTS: CNS DIZZINESS, drowsiness, fatigue, EENT


hearing loss CV CHF, edema, GI hepatoxicity, nausea, abdominal pain,
anorexia, diarrhea, GU albuminuria, decreased libido, DERM toxic epidermal
necrolysis, photosensitivity,puritus, rash, Mis: allergic reaction including
anaphylaxis

Route: po oropharyngeal 200mg daily for 1-2 wks. oropharyngeal


candidiasis 100mg bid for at least 1-2 weeks

Assessment

assess for signs and symptoms of infection (vital signs, lung sounds, sputum,
WBC, oral and pharynageal mucosa,nail beds) before and periodically during
theraphy.

obtain specimens for c&s before theraphy.

Implementation

do not interchange capsules and oral solutions

administer with a full meal to minimize nausea or vomiting and to increase


absorption

do not administer with antacids or other meds. that may increase gastric ph.

Patient teaching

to take med as directed even if feeling better

take at same time each day
