Team registration form, along with the team's entry fee and individually signed consent waivers, due by
Tuesday, January 18th - 9pm at the K-Life Building (6th and Main). Teams registered between Wednesday,
January 19th and Saturday, January 29th are welcome to play in the tournament but will not receive a t-shirt.

Each player MUST have a signed consent waiver to participate – No Exceptions

Format: The tournament will be double elimination format.

*Each team will be required to have a 2 to 6 ratio (females/males).
Brackets will be determined by a random drawing, there is no seeding
Entry Fee: $15 per player (this includes t-shirt for those registering by the 18th of January).

**Optional - Extra Life Bands (for return to the game) available**

Purchased prior to game-1 for $5 or 3 for $10, $10 each during the game

Time: 9:00 am Check-In/Registration Begins for morning session (Elementary & Jr. Hi), 1:00 for afternoon
(High School, College & Adult)
9:30 am Captains Meeting, 1:30 for afternoon session
10:00 am Games Begin for morning session, 2:00 for afternoon

**There will be a concession stand with lunch and other snacks available.
**Prizes awarded for the best-dressed team. We are excited to see your team spirit!
