Client's Profile

Client A was born on February, 1977 at the province of Davao del Sur
City and is presently 33 years old. He resides at Kiblawan, Davao del Sur City.
He weights 54.5kg and stands 5'5”. He is a Single and a Roman Catholic. Client
A is a military personnel. He was admitted Aug 5, 2010 at around 10:30 in the
morning.

History of Present Illness

1 year PTA, when Patient experienced weakness, generalized with easy fatigability. Patient
tolerated his condition and did not seek any consultation. Patient also noted slight enlargement of
anterior neck area. No medications was taken.

6 months PTA, Patient noted persistence of above symptoms, associated w/ weight loss,
increased appetite. The anterior neck mass was also noted to increase in size. Now associated w/
dysphagia. Still no consultation was done and no medications was taken.

3 months PTA, Patient noted to have excessive sweating w/ preference for cold
environment. Patient still noted easy fatigability and palpitations. Patient sought consult at Cotabato
Station Hospital where he was admitted and managed as a case of DTG. Patient was transfered to
AFPMC then again transferred to FBGH due to lack of bed space.

Past Medical History

Patient has no major childhood illnesses except for common cough and
colds. He has neither food nor drug allergies. According to him, he completed
all the necessary immunizations when he was young. This is the first time that
Patient was admitted to the hospital, meaning he also has no previous
surgeries or injuries.
