I, Subramanya Medidi do hear by solemnly state and affirm as hereunder:

1. I am Over twenty-one years of age and otherwise competent to make this affidavit.

2. From Dec, 2009 to till date, I am working for Verizon , 919 Hidden Ridge ,lrving,TX
75038 as a Programmer / Analyst and during this period Mr. Mohan Vuppalapati is
Working as a programmer Analyst here at Verizon as my co-worker to my knowledge no
Less than 40 hours per week.

3. I certify that to my knowledge Mr. Mohan Vuppalapati performed the following duties :

 Involved in complete Software Development Lifecycle (SDLC). o Coordinate with business

users in gathering business requirements and translating them to technical specifications.
 Developed Stored Procedures and User Defined Functions to migrate the data from
the Legacy system in the Destination tables.' Extensive use of T-Sql in creating Database
Objects and Loading the data from the source.
 Created complex SSIS Packages for bulk load and data transformations using
Microsoft Bl Tools.
 Migrated the Database and Database objects from Sql Server 2005 to Sql server
2008.

 Worked on Performance tuning the Stored Procedures and the SSIS packages.
 lnvolved in unit testing ,load testing and performance testing. o Co-ordinate with DBA,
Development and Management teams to ensure the Database and development work is
going along with the code.
 Ensure the architectural design is in compliance with the lT methodologies and client
requirements.
