Guidelines in Applying for NBI Clerance

Following are the guidelines in connection with the request for issuance of NBI Clearance: 1. Request for the NBI-Fingerprint Card Form from the Embassy/Consulate by providing the address of the applicant; 2. Fill up the personal data called for on both sides of the card form truthfully and legibly; 3. Request the assistance of the nearest police station / agency in having your fingerprints impressed (ROLLED IMPRESSION, please see sample on the next page) on the appropriate spaces. The person who fingerprinted you should sign his name and state his official designation on the space provided for the purpose in the card. Stamp the seal/identity of the police agency and/or station on the NBI fingerprint card form; 4. In case of married women, the order of writing the name is as follows: a. Your maiden surname (family or surname of your father) b. Your first name or given name c. Your maternal surname (family name of your mother) d. Your husbands last or family name and first name (space for this purpose is provided on the card) 5. Attached a latest copy of your black and white photograph (2 X 2) or passport size taken within three (3) months before this application at the appropriate box in the NBI fingerprint card form. Upon completion of these requirements, please send the form to the nearest Philippine Embassy / Consulate together with the following additional requirements: a. Self-addressed return envelope for mailing b. Payment of A$45.00 either by bank cheque or money order c. Photocopy of passport to be stamp/signed by a Justice of the Peace d. Provide address and contact details The card form with the seal of the Philippine Embassy / Consulate will be returned to the applicant for the applicant to send it either directly to the NBI office in the Philippines or to any relatives/friends who can apply for the renewal on behalf of the applicant. If the application will be sent directly to the NBI office, the payment will either be by International Bank Cheque or Money Order the sum of (converted to Pesos) TWO HUNDRED PESOS (P200.00), payable to the Director, National Bureau of Investigation, and should be sent on the address below: NBI Clearance Center Carriedo Plaza Building Carriedo St. cor Estero Cegado and Sales St. Quiapo, Manila Tel. no. 734-4907, 734-2922
