18 PURSUANT TO THE STIPULATION OF THE PARTIES HEREIN, THE COURT

19 ORDERS, ADJUDGES AND DECREES AS FOLLOWS:

1. It is further ordered that the care, custody and control of the minor children of the

parties, namely: NAME OF MINOR CHILD, born DATE OF BIRTH, shall be as follows: Legal

and Physical Custody to Petitioner. Said custody is subject to the right of reasonable visitation which

24 is hereby awarded to Respondent according to the following terms and conditions:

25 A. In odd-numbered years commencing in 20__, the entirety of the Christmas school holidays
26 from noon on the first saturday following the last day of school to noon on the day before school

resumes in January, in even-numbered years commencing in 20__, from noon on December 26 to

 1 noon on the day before school resumes in January.
2 B. The entirety of the spring school holidays from noon on the first Saturday following the

last day of school to noon on the day before school resumes.

C. The first six weeks of the summer school vacation between noon on the Sunday following

the last day of school in the spring semester to noon Saturday of the sixth week thereafter.

7 D. Alternate holidays weekends commencing with Columbus Day 20__ upon one weeks

8 prior notice to the Petitioner (such alternating holiday weekends to not include Easter, the Fourth of

July, Labor Day, Christmas or New Years). For the purposes of this subparagraph holiday
10
weekends are defined as three to five day periods including a Saturday and Sunday and a legal
11
holiday recognized and celebrated by the school in which the children are enrolled. Father may have
12

13 visitation at other reasonable times upon 48 hours notice. All visitation under this subparagraph shall

14 be exercised within 200 miles of the childrens primary residence and the exchange shall take place at
15
an agreed upon location in or near ___________. The provisions of this subparagraph are entirely
16
optional for the Respondent and his failure to request visitation under this subparagraph shall not be
17
considered by any Court in any future modification proceeding.
