    Document Checklist

    I have read the information for new undergraduate students on the College website and am aware that I need to arrive on Tuesday 4th October and register with the Academic Administrator that day. I am returning the following documents (please tick): College Contract University Contract please print double-sided and remember to attach a photograph. Two further passport photographs remember to write your name on the back! Tick this box if you dont want your photo included on the Freshers Faces page included in Freshers Packs (to help learn names) Junior Member Student Record Form Doctors Registration Form and Health Questionnaires please place in a sealed envelope for confidentiality. 200 College Deposit* please make cheques payable to Lincoln College, Oxford.
    * The College Deposit is refunded only when your course at Lincoln is complete

    On Tuesday 4th October there is a special (free!) welcome dinner for all undergraduate freshers. I will be coming to the Freshers Dinner on Tuesday 4th October (please tick)

    Please let us know of any dietary requirements:

    International Students only: Students travelling from overseas may arrive over the weekend of 1st/2nd October in order to attend the induction session for international students on 3rd October. I will be arriving on 1st October / 2nd October 2011 (delete as applicable). (It will be assumed that you will require accommodation from this date) Name: Course: Signature: Date:

    Please post this form and the accompanying documents to Jemma Underdown, Academic Administrator, Lincoln College, Turl Street, Oxford, OX1 3DR, to be received by Friday 9th September 2011. Make sure you attach the correct postage!!
