DIAGNOSES
Risk of communicable diseases among Madawon residents r/t overcrowding, poorly sanitation, and improper waste disposal.

RATIONALE

PLANNING

INTERVENTIONS
1.Assess degree of knowledge regarding disease transmission. 2. Conduct a health education that will tackle the following: a. proper handwashing b. proper hygiene c. proper waste disposal d. proper waste segregation e. precautionary measures to prevent disease transmission 3. Attend evacuees needs (food, drinking water, clothing and toileting in collaboration with other organization (LGU, NGOs) 4. Provide treatment for the reported cases of cough and colds, diarrhea and other diseases in collaboration with the other health care personnel, LGU, and NGOs 5. Provide emotional and psychological supports to families

RATIONALE

OUTCOME EVALUATION
-Goal met. -People in the community verbalize understandin g and are ready to apply all the preventive measures on how to reduce transmission of communicabl e diseases.

Diseases are highly transmissible in a populated area due to greater tendency of direct contact especially airborne transmission.

After two days of nursing intervention, the community is expected to observe proper ways on how to prevent communicabl e diseases.

-Determine level of understanding among the evacuees. -Increase level of awareness on the prevention of communicable diseases.

-Improve physical and nutritional status of evacuees to combat diseases. - Reduce or minimize the number of cases in the evacuation center. -Help the evacuees cope up positively with their present situation thus reducing PTSD.
