peacefully joined his Creator on September 1, 2012 at the age of 69.
His loving wife CLAIRE; Children David & Marga, Jomar, Clariza and Mark; Grandchild Lucas Javier, Mother Salvacion; Brothers Salvador Jr. and Leonardo; Sisters Digna and Catalina; in-laws, other relatives and friends request your prayers for the eternal repose of his soul. His body lies in state at the Montserrat Chapel, Loyola Memorial Chapels, Sucat. Interment details to be announced later.

Procurement of Information and Communications Technology Equipment for DOH-ARMM Package 8


1. The Government of the Philippines (GOP) has received a Grant from the European Commission being administered by the World Bank toward the cost of Health Sector Reform Project, and it intends to apply part of the proceeds of this Grant to payments under the contract for Procurement of Information and Communications Technology Equipment for DOHARMM Package 8. The Department of Health (DOH) now invites bids for Procurement of Information and Communications Technology Equipment for DOHARMM Package 8. Delivery of the Goods is required within thirty (30) days upon receipt of the Notice to Proceed (NTP). Bidders should have completed, within three (3) years from the date of submission and receipt of bids, a contract similar to the Project. The description of an eligible bidder is contained in the Bidding Documents, particularly, in Section II. Instructions to Bidders. Bidding will be conducted in accordance with relevant procedures for open competitive bidding as specified in the Implementing Rules and Regulations (IRR) of Republic Act (RA) 9184, otherwise known as the Government Procurement Reform Act, with some amendments, as stated in these Bidding Documents and is open to all Bidders from eligible source countries as defined in the applicable procurement guidelines of the World Bank. The contract shall be awarded to the Lowest Calculated Responsive Bidder (LCRB) who was determined as such during postqualification. The Estimated Project Cost (EPC) is Two Million Eight Hundred Ninety Six Thousand Philippine Pesos (PHP2,896,000.00)

INVITATION TO BID

Bidders should have completed, within Five (5) years from the date of submission and receipt of bids, a similar contract costing at least fty percent (50%) of the project. The description of an eligible bidder is contained in the Bidding Documents, particularly, in Section II. Instructions to Bidders. Bidding will be conducted through open competitive bidding procedures using a non-discretionary pass/fail criterion as specied in the Implementing Rules and Regulations (IRR) of Republic Act (RA) 9184, otherwise known as the Government Procurement Reform Act. Bidding is restricted to Filipino citizens/sole proprietorships, partnerships, or organizations with at least sixty percent (60%) interest or outstanding capital stock belonging to citizens of the Philippines, and to citizens or organizations of a country the laws or regulations of which grant similar rights or privileges to Filipino citizens, pursuant to RA 5183 and subject to Commonwealth Act 138.
