Shoe Hire Everyone will need a pair of black jazz shoes. Girls may need a pair of black character shoes.

I would like to hire shoes for my child for the production. I understand that you will size my child for the required shoes and enclose my payment of 5 (payable now) for the hire fee. We will size them for you at rehearsal. I do not want to hire shoes and will provide them myself. I understand that the shoes must be named and left at the theatre during the week of the production.

Face Paint We use Snazaroo face paint for the production. They are high quality, hypoallergenic, non-toxic and easy to remove. You can find our more about them here: www.snazaroo.com.

Download and read the Guide (www.themaninthemoon.co.uk/production). Complete both the Licence Application and Medical Questionnaire (www.themaninthemoon.co.uk/licence). Enclose a copy of your childs Birth Certificate (or other satisfactory evidence of childs age). Enclose two identical, recent (within the last 6 months), passport size (35mm x 45mm) photographs. Payment of 5 (cash or cheque payable to MIM) for shoe hire (if required). Enclose your 5 returnable script deposit (if required). Cheque made payable to MIM, no cash please.
