1.0 FORWARD These Guidelines serve as a practical guide to Health, Safety and Environment (HSE) with the objective of minimizing hazards at workplace, maintaining ZERO Lost Time Injuries and protecting and preserve of the environment. These Guidelines shall assist all employees develop high HSE awareness and understand their responsibility for HSE. A low accident frequency 2.0 HEALTH, SAFETY AND ENVIRONMENT POLICY It is XXX Ventures philosophy to create a safe, health and conducive work environment for all our employees with equal importance to the safety and health and of the general public, and with the emphasis that work must be tailored to include safety, health and the environment as the principle objective. We will comply to all applicable safety and health laws and regulation as in OSHA 1994 (ACT 514) and FMA 1967 (ACT 139) and will take every possible measure to prevent job related injuries and illness.

It is our explicit intention that every employee cultivates sound safety and health practices as an essential ingredient to the overall success of our business and human resources.

5.0 RESPONSIBILITIES AND FUNCTIONS The responsibilities and functions of Project Management, supervisory personnel and workforce are described bellows: 5.1 Project Manager The Project Manager is to ensure that the objective of XXX Ventures Sdn Bhd Health, Safety and Environment Policy are complied with. The necessary support is provided to enable site personnel to implement all aspects of XXXS Health, Safety and Environment Policy and procedures within the PROJECT projects. The Project Manager must familiar with the relevant Legislation and Codes of Practice. The Project Manager is accountable for the overall Health, Safety and Environment performance of the Project including: Liaise closely with the Construction Manager and HSE Supervisory Personnel in the implementation of the Project HSE Management Programme. Develop, enforce and review safe working procedures and systems. Attend the Clients Site HSE Management Meeting. To became a Project HSE Committee Chairman and chair the meeting as per scheduled. Immediate rectification and/or disciplinary action as a result of any breaches in Site Health, Safety and Environment Procedures or an unsafe working environment Schedule work programmes with the HSE of personnel given highest priority Encourage and advise supervisory personnel that HSE inspections are an essential tool in prevention strategy

5.2 Construction Manager Constructions Manager has the following tasks: To assist Project Manager in implementation of Construction HSE Plan and take fully charge on project management during the absent of project manager; Liaise closely with the Project Manager and HSE Supervisory Personnel in the implementation of the Project HSE Management Programme. To ensure all construction employees compliance to this procedures while discharging their construction activities. Any substandard conditions or acts noted, he initiates the necessary corrective actions; Participating in the weekly walkabout and Monthly HSE Committee Inspection and meeting; To become HSE Committee Chairman during the absent of Project Manager or on out station duty; Participating in accident investigation and develop recommendation to prevent recurrence

5.3 Supervisory Personnel (Engineers, Supervisor, Foreman, work leader etc) The supervisory personnel has the following duties and responsibilities: Are responsible for all employees under their supervision and the care and maintenance of a safe working environment. Liaise daily with the Project Manager, Construction Manager and HSE Officer in the implementation of the Project Site HSE Implementation Programme. Required to develop the Method of Statement and Job Safety Analysis for dangerous activities. Ensure that all employees adhere to the Clients HSE Procedures as well as the XXX Project HSE Site procedures. Required to take part in weekly joint walkabout. Required to organise toolbox meetings daily. Maintain safe equipments, tools, environments. Arrange adequate storage and enforce good housekeeping. Ensure Permit to Work are approved prior to perform any dangerous activities.

Discourage members of their work groups from taking risks. Create an attitude of safe work behaviour and maintain it. Conduct regular work place inspections, ensuring that prompt action is taken on problems that arise. Report all incident/accident immediately to Construction Manager or HSE Manager or Officer that occurs to an employee under their supervision. Familiarize with the workplace emergency evacuation plan and take part in the rescue exercise during emergency. Lead by personal example.

5.4 HSE Manager The incumbent shall: Be the Management Representative on all HSE matters. Effectively manage the companys HSE Department. Enforces compliance of company and statutory regulations procedures and practices by employees and subcontractors. Develop new procedures to enhance HSE in the workplace. Investigate accidents with personal injury or property damage, incidents and near misses and write corresponding reports, compile the corrective action request log, compile monthly HSE performance statistic for the whole construction site. Implement the Companys/Projects HSE program Make regular walkabout on project site and provide appropriate measures to be taken on the substandard condition and act noted during walkabout to Project Manager for further action. Train and motivate employees for safe behaviour. Cooperate with Clients and Subcontractors HSE Manager or Engineers. Participate in Project HSE Committee Meeting.

5.5 HSE Officer It shall be the duty of a Safety & Health Officer to advise the employer or any person in charge of a place of work on the measures to be taken in the interests of the safety and health of the persons employed in the place of work; to inspect the place of work to determine whether any machinery, plant/project site, equipment, substance, appliance or process or any description of manual labour used in

the place of work, is of such nature liable to cause bodily injury to any person working in the place of work; to investigate any accident, near-miss accident, dangerous occurrence, occupational poisoning or occupational diseases which has happened in the place of work; to assist the employer or the safety and health committee, if any, pursuant to regulation 11 of the Occupational Safety and Health (Safety and Health Committee) Regulations 1996 in organizing and implementing occupational safety and health programme at the place of work; to become secretary to the safety and health, if any, as specified under subregulation 6(2) of the Occupational Safety and Health (Safety and Health Committee) Regulations 1996, and perform all functions of secretary as specified in that Regulations; to assist the safety and health committee in any inspection of the place of work for the purpose of checking the effectiveness and efficacy of any measures taken in compliance with the Act or any regulations made under the Act; to collect, analyse and maintain statistics on any accident, dangerous occurrence, occupational poisoning and occupational disease which have occurred at the place of work; to assist any officer in carrying out his duty under the Act or any regulations made under the Act; and to carry out any other instruction made by the employer or any person in charge of the place of work on any matters pertaining to safety and health of the place of work.

5.6 Site HSE Supervisor The duties of site HSE supervisor as follows: Ensure that the provisions of the Act and any regulations made thereunder are complied with. Promote the safe conduct of work generally within the worksite. Inspecting and rectifying any unsafe place of work. Correcting any unsafe practices Checking sub-contractors work to ensure compliance with the Act and the regulations made thereunder. To liaise with subcontractors HSE supervisor or project management team on the implementation of HSE programme. Liaise closely with Project HSE Officer on all matters related to project health, safety and environment.

It shall be the duty of every employee while discharging their work activities: To taken reasonable care for the safety and health of himself and of other persons who may be affected by his acts or omissions at work. To cooperate with his employer or any other person in the discharge of any duty or requirement imposed on the employer or that other person by the Act or any regulation made thereunder. To wear or use at all times any protective equipment or clothing provided by the employer for the purpose of preventing risks to his safety and health. To comply with any instruction or measure on health, safety and environment instituted by his employer or any other person by or under the Act or any regulation made thereunder. Report any incident/accident or injury which were occurs within their vicinity. Ensure that they use the correct tools and equipment and report any defective equipments/tools to their supervisor or foreman. Actively participate in Toolbox meetings Familiarize with the workplace emergency evacuation response procedures

5.8 Subcontractor Responsibilities The subcontractor is responsible for the following: Observe and comply with regulations and standards on HSE in line with provisions of this Site Safety Procedure and understand their responsibility for HSE. Conduct regular inspection on all tools and equipment and ensure that they are in safe and operable conditions. Conduct daily five minutes HSE toolbox talk and ensure the working areas are safe and free from hazards prior to work commencement. Ensure that their new employees have attended Main Gate HSE induction and possess CIDB and Site identification card. Provide employees with appropriate personal protective equipment to meet job requirements and replace as necessary. Ensure that equipment and tools brought on to the site are of the required standard, approved and safe to operate. Ensure housekeeping in their respective working area is properly maintained and arranged the disposal of all unnecessary materials. Always adhere to HSE requirements whilst performing their job and respond positively in an emergency.

Ensure equipment is shut down before leaving site or on completion of work. Participate actively in all HSE activities such meetings and joint HSE inspections

5.9 HSE Committee A Safety & Health Committee shall meet as often as may be necessary commensurate with the risks attendant on the nature of work at the place of work but shall not meet less than once in three month (Occupational Safety and Health Act 1994 (Safety and Health Committee Part III Regulations 11 ) Regulations 1996. The Project HSE Committee set up as follows (Refer Appendix I): Chairman Project Manager Secretary Safety & Health Officer 4 Members from Employer Representative 4 Members from Employee Representative

The safety and Health committee of a place of work shall: Assist in the development of safety and rules and safe system of work. Review the effectiveness of safety and health programmes. Carry out studies on the trends of accident, near-miss accident, dangerous occurrence, occupational poisoning or occupational diseases which occurs at the place of work, and shall report to the employer/ any persons in-charge of the place of work of any unsafe or unhealthy condition or practices at the place of work together with recommendations for corrective actions. Inspect the place of work and make recommendations to the employer or any persons incharge on the remedial measures to be taken on any matter prejudicial to health, safety and environment of persons at the place of work. Participate in the incident/accident investigation and develop recommendation to prevent recurrence and follow up the effectiveness of the accident prevention programme.

6.0 GENERAL SITE HSE RULES AND REGULATIONS Introduction


XXX Ventures shall ensure that each subcontractor is provided with a copy of these basic project site HSE Rules and Regulations. Each employee is required to know and to comply with the applicable HSE rules and regulations of the company. Any employee, who does not comply with these HSE rules and regulations as stated in this section and/or his supervisor's instructions shall be removed from the site. 6.1 Smoking, Alcohol and Drugs Policy

No Smoking signs must be obeyed, smoking is only permitted in designated smoking areas. Hazardous areas are designated No Smoking . No alcoholic beverage or non prescription drugs are permitted within the place of work. It is not permitted to consume, be in possession of or be under the influence of any alcohol or drug product. Disciplinary action up to and including dismissal may be taken against any employee failing to comply with this HSE requirement. 6.2 Personal Protective Equipment (PPE) All supervisory personnel at site shall ensure all employees working at site wear the appropriate PPE required for the job or location to enable them to perform their duties safely. Steps must be taken to select the proper type of PPE, ensure that the supervisor properly instructs his employees in the use of that equipment and the availability of and adequate stock/supply of PPE on site. This shall also apply to PPE for visitors. All PPE must comply with at least local standards and regulations. For all work to be carried out on the construction site the following PPE shall be used as a minimum: Appropriate working clothes for the individual job, dressed upper part of the body, no sleeveless upper body wear, and no shorts. Cotton material shall be used throughout. Hard Hat have to be worn at all times on the whole construction site except within the office areas and within vehicles. Hard hats should be scrapped following any penetration, high impact or according to the manufacturers instruction and expiration date. Safety Spectacles with side shields have to be worn at all times on the whole construction site except within the office areas and within vehicles. Safety shoes or safety boots have to be worn at all times on the whole construction site except within the office areas.

These requirements are valid also for delivery services. A staff member in charge shall accompany visitors provided with hard hat, safety spectacle and safety shoes or safety boots. For further protection against hazards, which cannot be controlled by any other technical means, specific PPE may have to be used according the requirements. Example are: Eye protection for welding, grinding or gas cutting. Face shields to protect face and neck from flying particles or sprays of hazardous liquids. Dust Masks, breathing masks, self contained breathing apparatus (SCBA). Gloves (not to be used near rotating equipment). Hearing protection at the area where the noise level is exceed 80 dB(A) Personal fall protection equipment (safety harness with shock absorber and double lanyard)

Cellular/Mobile Phones can be used on the construction site in the following areas only: Offices areas Lockers areas Parking areas Main Gate area

The use of mobile phones in all other areas is prohibited unless approved in writing by Construction Manager. 6.4 Vehicles, Machinery and Traffic Control Vehicles and machinery such as cranes, pickups, backhoe, loader, lorry, trucks, forklifts, graders, compactors, generators, pumps, compressors shall be regularly inspected by the user prior to use. All operators of the vehicle or machinery must have valid operators license issued by the authority. Appropriate operating documents shall be available and valid. All mobile vehicles and machinery shall be properly parked within the project site, and if situation warrants due to congestion, the mobile vehicle machinery shall be instructed to be parked outside the project site. Private vehicles such as cars and motorbikes shall not be allowed into the project site. A parking area adjacent to the main gate and near to the project site office shall be dedicated for parking of private vehicles. Company or subcontractor vehicles to be used on the site shall be equipped with fire extinguishers suitable for the expected need and the speed limits on site is 20 km/h Smoking or using a mobile phone while driving is strictly prohibited. 6.5 Job Safety Analysis (JSA) Job Safety Analysis shall be prepared for critical activities such as heavy lifting, lifting using multiple cranes, blowing, hydrostatic testing and flushing activities, and other hazardous construction and/or pre-commissioning activities. The responsible supervisor shall study the basic job steps, identify the hazards and propose mitigating measures to eliminate the hazards and safety precautions to be instituted. The JSA shall be reviewed and approved by the Area Owner and Project Management Team for the use during the execution of the work. 6.6 Work Permit System Owners PTW system shall be complied to ensure control of work within / near operating / energised facilities, within confined spaces, and other hazardous environment. PTW shall ensure implementation of safe work rules and procedures and practices that include issuing of permits that authorises such work. The following are the categories of the permits: Hot Work Permit Cold Work Permit Certificate associated to PTW

The Owner Representative shall train supervisory personnel, especially Work Leader, in the above PTW requirement. During practical application of the work permit system the Subcontractor has to make sure the following: Specify the work to be performed, the required employees and the equipment to be used The precautions to be taken are specified by completing the corresponding permit form. Client or Contractor gives permission to start work through signing the permit form that must be retained at the workplace. Only the employees named therein are allowed to perform the work. Prepare the work under consideration of the safety precautions specified in the work permit, instruct employees and hand over necessary tools and PPE. Finally check all precautions, carry out necessary gas tests and perform work. After completion of work or daily at end of shift the work permit must be returned to issuer.

A permit cannot be considered as an unconditional warranty for safe job execution. If the conditions, under which work permits have been issued change, Subcontractor shall stop the work and report the changed conditions to Contractors Discipline Supervisor. Besides, if start of work is delayed or work interrupted for more than two hours for other reasons than process safety considerations, the Subcontractor shall inform Contractors Discipline Supervisor prior to resume work. Client or Contractor may, at any time, invalidate a work permit due to changed conditions, especially in case of an emergency. 6.7 Excavation and Trenching The following requirements shall be adhered during excavation & trenching activities: Excavated materials and other superimposed loads shall place at least 1 m from the edge of open excavation and trenches. Excavated materials/soil left overnight shall be protected with plastic to prevent from flowing back into the trench or road during heavy rain. No employees shall be permitted to enter any excavated area unless sheet piling, shoring or other safeguards for his protection are provided. Every excavation of more than 1.2 m depth shall be provided ladders, stairways or ramps to furnish safe access to an egress from such excavation and shall be installed in a sufficient numbers and in such locations as to be readily accessible. Where trenching of more than 1.5 m in depth, pilings, shoring, bracing or other safer method shall be used to protect employees against falling or sliding materials. Where trench to be excavated exceeds 4 m in depth, such protection shall be constructed in accordance with the design and drawings of a Professional Engineer. The trench shall be sufficient size to accommodate the activities and number of person in the trench. No person is allowed to work alone in the trench. In case of backfilled was not completed, the trench shall be barricaded with reflector tape and blinkers are essential to warn people during darkness. The trench shall be protected by interlocking plastic barriers to prevent from any vehicle get into it.

6.8 Hazard Warning and Communication Adequate safety signs, signboards, notices, and barricades shall be placed by Contractor or its subcontractors to restrict access to potentially dangerous areas and also to notify personnel of specific hazards. Supervisors shall be responsible in implementing this requirement with assistance from the Project Supervisor. 6.9 Portable Tools and Equipment All electric powered and air powered tools and equipment to be used at site must be inspected jointly by supervisory personnel and user prior to use. Personnel using the tools and equipment must be adequately trained in the proper and safe use of the tool and equipment. Appropriate PPE as per the job shall be provided and wear whenever necessary. Work Leader/ Site Supervisors shall be responsible to ensure all hand tools and equipments are safe and good working conditions prior to use.

6.10 Fire Protection As minimum the following fire protection rules have to be observed on site: Smoking is prohibited on the premises, also within vehicles. Excluded are designated smoking areas. Open fires, open electrical heaters, radiators etc. are prohibited. Defective isolation of electrical tools, leaking gas pipeline, vessels or fittings have to be reported to Work Leader/Supervisor without delay. Beware of flammable materials or vapours in ducts or drains. Subcontractor has to take all appropriate measures for fire protection during the entire project. He has to place signs in front of areas with special fire hazards and to supply sufficient fire protection equipment to all areas where the fire hazards are exist. Fire hazardous work shall be carried out only after the necessary precautions have been taken (e.g. fire extinguishers, water buckets, fire blankets against sparks etc.). A stand-by person (fire post) might be necessary. Any accident caused by fire shall be reported to the Work Leader/Supervisor for necessary actions. If possible without endangering people fire fighting equipment should be used in order to extinguish the fire at early stage. 6.11 Handling of Flammable Liquids Flammable Liquids on construction site are e.g. gasoline, diesel, mineral oil, grease, used oils, liquefied gases, thinners, lacquers, glues, adhesives etc. Besides legal requirements the following rules must be observed as a minimum when handling flammable liquids: Make sure proper ventilation.

Suck off flammable vapours with suitable explosion proof equipment. Avoid any ignition sources (also non explosion proof electrical tools). Use only non sparking and explosion proof tools. Avoid open containers. Do not use flammable liquids for cleaning purposes (clothing, machinery, floors etc.). Take precautions against static electricity. Avoid any spills when filling. Have absorbents in place. Have suitable fire extinguishing equipments in place. 6.12 Lifting, Rigging and Material Handling All lifting activities shall be planned ahead of time to ensure smooth and safe work co-ordination. Lift study to include job safety analysis shall be developed prior to critical lifting activities such as heavy lifting above 100 tonnes, combined lifting using 2 or more cranes, lifting in congested areas. Only trained and licensed or DOSH Certification Competent person shall operate cranes. Prior notification to all parties and proper communication shall be made to ensure safe lifting. The Lifting Supervisor shall be responsible to ensure all the lifting equipments and tools such as rope, slings, chains, shackles, webbing slings and other devices are from wear and tear. Trained Banks man/signalman shall be assigned to assist crane operator during lifting and rigging activities.

6.13 Storage
A dedicated storage area shall be built outside project construction area for storage of construction materials. Good Storage practices shall be maintained at all times. Proper segregation of material shall also be practiced to ensure safety and compatibility of hazardous material especially flammables / combustibles, chemicals and compressed gases which must be stored separately. Only authorised personnel shall be allowed in the storage facilities. The Store Keeper shall keep where appropriate the CSDS of chemicals for references. All new lifting gears shall register the lifting gears inspection record sheet for references. 6.14 Work at Height The followings are the rules and regulations that required to be followed when working on height 2.0 meters and above: Appropriate working platform to be erected, tested and verified safe to use by DOSH approved erecting companies (Scaffolding Erection Company). Any modifications of scaffolds by the user are strictly prohibited. Any scaffolding without green scaffold tag displayed are not allowed to use.

Guard rails or fall protection for all temporary working platform or scaffolding to be installed and ensure that they are safe to use. Fall arrest protection or full body safety harness are have to be provided and ensure that all personnel working on height at the areas where the falling hazards are exist. A Proper access to all scaffolds or temporary working platform has to be provided and ensure that they are safe and free from any obstruction.

6.15 Blasting and Painting Blasting and Painting shall only be done in a specified zone, and if possible away from other construction activities due to the high noise emitted by the air compressors. Only trained, experienced and competent personnel shall operate blasting and painting equipment. Adequate PPE such as life line breathing suit, ear muffs, Blasters helmet, leather gloves / aprons shall be used. The Supervisor shall be responsible to ensure the blasting/painting equipments are in good working conditions prior to use. Appropriate warning sign to be displayed at the blasting/painting area. Air Supply shall be free from Carbon Monoxide (CO). During blasting/painting works, a standby man shall be provided to warn operator in the event of any hazard occurring while the equipment is in use. 6.16 Radiography Radiography Operator shall ensure radiography accident prevention are well managed prior to perform radiography test at site as per LPTA requirements Radiography Operator is responsible to obtain the LPTA permit before RT activities. 6.17 Emergency Response Plan In the event of major incident/accident occurring at the place of work such fire and explosion, bomb trap, earth quake, strong wind, heavy flooding and any accident that harmful cause by workplace violence, the following emergency evacuation response procedures must be followed: Any employee at the location can immediately contact the site engineer/supervisor and safety & health officer or raised emergency alarm (if any). This information have the advantage of allowing them on the earliest possible action to be taken to evacuate, rescue and control the situation. An emergency contact will be displayed at the strategic location at the site. All workers in the vicinity are required to be assemble at the designated Assembly Area until further advise by the Emergency Commander (Construction Manager/Site Engineer)

6.18 Incident/Accident Reporting System All incident/accident shall be reported immediately to Project Manager/Construction Manager/SHO and Owners Representative. Site Supervisor/Work Leader concerned must complete the Owners Initial Site Incident Report Form (GMB 6069E) and submit the report to relevant parties with 24 hours and than the Project/Construction Manager will form a investigation team to carried the investigation and the report should produce with 48 hours.

The followings are the safety precaution need to be followed during tie-in operation:
No smoking within tie-in vicinity. Designated Smoking Area to be assigned at each site. All electrical and electronic equipment used on site to be either explosion proof or intrinsically safe. Examples include hand-held torches, instrument and power tools. All vehicles to be park at the temporary designated parking area. All access to be kept free from any obstruction during purging operations to ensure rapid escape in event of emergency. Fire extinguisher (9 kg Dry Powder ABC type) to be located adjacent to venting point while in used. Fire extinguisher to be on site at each skid location. Additional protection such as fire blanket to be provided. First Aid Box to be available on site being commissioned and moved as job progresses. Certified First-Aider to be stationed at site for immediate first-aid treatment during emergency.

Gas Warning barricades to be erected around each mainline venting pit and at entrance. In additional preferably sign to segment barricades stating: TIE-IN IN PROGRESS. Prior to gas-in, inform the Local Authority such Fire & Rescue Department, Police and Hospital for their assistance required during emergency. Specific Gas Venting Hazard to be observed: Flammable and explosive mixtures may be venting High velocity gas can course injury when impacting the body or may permeate the skin and enter circulatory system.

Leak response action: Shut off pressure source (e.g. Nitrogen bottle or pipeline) Depressor leaking pipes work. Repair leak in-situation if possible (e.g. tighten flange) If component to be removed from system, isolate from natural gas source by double block/bleed system or insert line spade.

Commissioning It is the responsible of each person engaged commissioning to take reasonable care for the health and safety of him and other person who be affected by his act or missions. Commissioning Commander is responsible to ensure accident prevention during commissioning are taken accordingly. He also required to brief JSA requirements to all commissioning team personnel prior to commissioning operation. The Safety and Health Officer is responsible to assist commissioning commander on ensuring site safety preparation such as safety signs, safety equipments and permit to work system is approved by approval authority.

Conduct Site Safety Briefing to all commissioning workers prior to commissioning operation and ensure all workers understand on emergency evacuation procedures.

6.20 Housekeeping Housekeeping shall be carried out daily before leaving the work place/site. All waste materials including but not limited to construction waste, office waste, litter and garbage shall be gathered without delay in strategically located disposal containers and continuously removed from the construction site. 7.0 ENVIRONMENTAL PROTECTION All local regulations on environmental protection (in particular those regarding emissions to air, water and soil and other waste materials) is to be observed and comply with: Air pollution caused by smoke, soot, dust, gases, aerosols, vapours and odorous substances has to be prevented or reduced to a minimum by way of measures relating to the process and application in question wherever this is reasonably possible. The following protective measures are listed as examples of how to reduce air pollution: Switch off heating, combustion engines and other consumers wherever possible. Keep Combustion Engines in good condition, especially calibration and synchronization of diesel engine injectors. Use low-solvent agents. For all work involving air-polluting substances, ensure good natural or artificial venting, in particular in closed locations. This will also reduce fire and explosion hazards and promote health protection. Use recommended PPE, particularly for installation of thermal insulation and grid blasting operations. As far as possible minimize areas exposed to airborne fibres and dust. Use screens, habitats, extractors and filter where possible. Limit access of this areas to essential personnel only, by use of safety barricades. Drive very slowly when using dusty roads. Use welding fumes suction systems with approved, adequate filters. The welding fumes that form during the welding of high-alloy materials (e.g. chromium nickel steels) may affect the respiratory organs and cause cancer.

The noise level has to monitor closely during construction activities. Noise prevention shall be taken accordingly if the noise level exceeded the local admissible limit values. The following protective measures are example for noise prevention: Ensure that powered machines and combustion engines are switched off and/or their speed is reduced wherever possible. The Contractor will define suitable access roads for site vehicles under noise considerations for the site and its surrounding areas. Set up noise protection walls around stationary workplace for grinding and adjusting work.

Ensure that existing noise protection hoods are not removed from machines and vehicles. Let your employees take noise breaks whenever possible. Hearing problems can be prevented but not cured. Reduce vibrations, in particular low-frequency vibrations, to absolute minimum required.

Almost all materials used on site are water polluting and, therefore, have to be handled with greatest care. The following measures for water and soil protection have to be considered: Reduce the use and storage of water polluting substances to the absolute minimum required. For diesel, prefer delivery service rather than storage of large amount of diesel on site. Water polluting substances may under no circumstances enter water bodies, the sewage system or the soil, e.g. when cleaning machines, equipment and vehicles or when fuelling vehicles and machines. This also applies for waste, in particular. Spillage shall be strictly contained. Ensure that all containers, cans, drums and others are placed in collecting basins and protected against mechanical damage (e.g. by collision with vehicle). Never store such containers at open space, passageways or directly at the workplaces Only use tanks with functioning overflow protection. Take defective pumps, connecting lines, couplings sections, etc. out of service. Have machines, etc. checked for oil leakage at regular intervals and immediately repair any defects found. Ensure that spillage equipment and material is available (e.g. absorbents, mobile collecting basins, sewage covers, pumps). Even small amounts of spilled, water polluting substances must be removed immediately. High-pressure cleaners only be used in plants equipped with a sludge retainer and an adequate downstream waste-water treatment unit. Whenever possible, have vehicles cleaned in a public car wash. Oversize equipment should be cleaned by companies with approved washing areas. Remove all foundations of temporary facilities after completion of the project.

General Waste Materials such as paper, cardboard boxes, plastics, scrap iron, wood, electrode residues and etc. to be collected and dispose at the designated disposal area for local authority disposal unit collection and further action.

8.0 HEALTH, SAFETY AND WELFARE 8.1 First Aid Facilities The employer responsibility with regards to first-aid requirements are as follows:

Providing and maintaining a first-aid box and ensuring the box is readily accessible at all times. Assigning the responsibility to upkeep a first-aid box to a responsible person, and for a place of work with more than 20 person employed, specifying that the responsible person should be proficient in first-aid treatment. Providing and maintaining a first-aid room in a place of work where more than 150 persons are employed. Provide refresher training to all certified first-aider.

8.2 Utility Facilities and Canteen Facilities for personal sanitation (toilets, washing facilities and lockers or places for drying and storing working clothes) should be provided near workplaces. Access to drinking water or other beverages and to canteens or other food services. Facilities to help reduce fatigue, such as seats, rest areas and smoking designated area. To assign someone with responsibility for keeping the toilets clean, and make sure that the cleaning is done everyday. To provide a place where short breaks can be taken close to the workplace, and canteens services are readily available.

9.0 SITE HSE PLAN AND PROGRAMME 9.1 Main Gate HSE Induction Briefing All Site Construction Supervisory Personnel and General Workers shall attend main gate HSE Induction briefing prior to work at construction site. This induction covers safe work practices, smoking policy and emergency evacuation response plan. A pocketbook will be issued to all attendees for their guidances. A part from the above all employees (Supervisory Personnel and General Site Construction workers) shall possess CIDB or NIOSH Coupon or NIOSH YYYYXX Safety Passport (NPSP, whenever required).

9.2 HSE Induction for Visitor All visitors shall report to Guard House for registration. The visitor organizer is responsible to ensure that their visitors attended main gate HSE induction briefing before taken them into the construction site area. This induction covers emergency evacuation response plan, general HSE requirements. 9.3 Meeting and Communication Kick-off-Meeting The kick-off meeting, held prior to the start actual construction site activities with all Subcontractors, will cover a view of the project activities, site HSE plan as well as other HSE

relevant issues. The Subcontractor HSE Obligations will be issued to subcontractor for endorsement by their Project Management. The meeting chairs by Project Manager. HSE Meeting Toolbox Meeting Toolbox meeting was held at the Owners Representative Site Office. Contractor and Subcontractor Project Management Team have to attend daily. The meeting was chaired by Owners or Owners Representative. The following subject were discussed in the meeting: Current work activities. HSE issues

Five Minutes Toolbox Talk To be conducted daily by site supervisor/work leader at site prior to commence work. The topic to discuss in the toolbox talked base on the findings noted during daily site supervision.

Weekly Toolbox Talk To be held on the 1st day of the week. All Project Management Team (e.g. Project Manager, Construction Manager, Safety & Health Officer, Site Supervisor) has to participate in this program.

Hand tools and Gas Cutting Equipment Inspection for Leakage To be carried out daily by user, Site HSE Supervisor to ensure all gas cutting equipments are in good working condition and free from any leakage.

Workplace HSE Inspection To be carried out daily by Site HSE Supervisor and Site Supervisor in order to ensure the workplaces are free from substandard condition and act. Any findings noted during this inspection shall be highlighted in the next morning five minutes toolbox talk.

Monthly Joint HSE Inspection and Meeting To be carried out monthly. This program is designed for Project HSE Committee Member to inspect the place of work. Any findings noted during this walkabout will be discussed in the meeting for further rectification action to prevent recurrence.

All site workers who are involved in the construction site activities has to attend the Safety Induction for Construction Worker. This training conducted by CIDB and a green card will be issued to all attendees upon completion of the training. Permit to Work System This training is designed for the PTW applicant. This PTW training was conducted by the owners representative at their site office. Only personnel who have the PTW authorization certificate are allow to apply the PTW for the hazardous job. Basic First Aid To be organised at site office for Project Supervisory Personnel who has yet to attend or certified first-aiders and those who due for refresher training. Other Training Other Trainings such NPSP, Entering into Confined Space, Crane Lifting will organise as and whenever required. 9.5 HSE Performance Report and Monitoring To be prepared and submitted to Owners Representative on monthly basis. All information pertaining to accident/incident, man-hours worked etc. as per listed in the Owners Monthly HSE Performance Report Form (GFA 0469E) are to be highlighted accordingly. 9.10 HSE Incentive To be provided upon completion of project without LTI.
