    1. Purchase a Philips Sonicare Toothbrush from Kohls Department Store between September 24, 2012 and November 21, 2012. 2. Enclose a clear copy of your original, dated sales receipt. Keep the original for your records. 3. Enclose the original UPC code cut from the bottom of the package. UPC codes that are handwritten or mechanically reproduced will not be accepted. 4.
