1. MEDICATIONS

Take all prescribed drugs in a prescribed dose, time , route and drug. Make sure that the drugs intake is continuous to the prescribed length of time and dont skip taking it. Encourage patients S.O to comply with the medication strictly following the right dose and route. Instruct and encourage significant others to monitor patient ability to take medication. Encourage patients SO to seek medical help if prescribed medications dont cause any change at all to alleviate patients condition if symptoms of the disease persists. Encourage the S.O to do frequent but gradual repositioning in bed to avoid pressure ulcers or skin irritations. Demonstrate and encourage the patient to do range of motion exercises with patients capabilities to promote good circulation. Encourage SO to take genuine medications from authorized drugstores.

Encourage patient to eat nutritious food and take well-balanced diet with the help of the SO. Encourage patient to increase oral fluid intake per day to maintain hydration and promote proper regulation of the body processes with the help of SO. Encourage patient to increase intake of carbohydrate-rich foods for faster regain of lost energy with SOs help.

4. HEALTH TEACHING

Provide with teaching regarding patients medication to SO to gain patients recovery. Discuss to SO the importance of taking a well-balanced diet to aid in patients recovery. Discuss to SO the importance of hygiene and sanitation to aid in patients recovery, as well.

5. SCHEDULE FOR THE NEXT VISIT

Encourage patient to have a visit to hospital if the patients condition upon discharge was not alleviated, with SOs help. Encourage patient and SO to return to the hospital as ordered by the physician.
