1. PURPOSE :
1.1 To ensure safe and competent drug administration based from Physicians Order.

2 DEFINITION : 2.1 Physician Orders are written documentation for the treatment plan and care of the patient.

3. POLICY : 3.1 Written Order : 3.1.1 All Physicians order must be legibly written on the Physician Order Form in the patient chart. It must be dated, timed, signed and include the Physicians Batch no. 3.1.2 Any abbreviations used must be on the Hospital Approved List. 3.1.3 Correction or change with the use of wipe out on the Physicians order is not allowed. A single line shall be drawn on it. The order must be re-written to reflect the change. 3.1.4 Medication Order must comply with prescribing guidelines with Hospital Pharmacy Policy and regulations. 3.15 After orders are written if / when possible should review all orders with assigned Nurse.

3.2 Telephone Order : ( Refer To P/P ) 3.2.1 Telephone Order should be limited to urgent situations where immediate written order is not possible.

3.2.2 Physician Order, must be sent to pharmacy, written that OR cancelled, resumed all previous medication(s) This order is accepted within the same day of OR cancellation. 3.3 Verbal Order : ( Refer P/P ) 3.3.1 Verbal Order is given to a Staff Nurse by a Physician who is physically present. These are reserved for Code / Emergency situation only. 3.4 Post -Surgery Order : 3.4.1 A post operative order that merely states review pre-operative order, pre-operative me dication is not acceptable. 3.5 Transfer Order : 3.5.1 An order for transfer must be written by the Physician prior to the patient transfer. 3.5.2 All medication order must be re -written when a patient transfer. 3.5.2.1 From one Unit to another. 3.5.2.2 From Operating Room to ICU. 3.5.2.3 From ICU to General Ward. 3.6 Renewal Order : 3.6.1 Renewal of routine medications, IVF must be written by the Physician. No telephone orders will be accepted for renewal of such.

4. PROCEDURES : 4.1 N/A.
