PROCEDURE TO BE FOLLOWED AFTER ACCIDENT. 1.

ACCIDENT INTIMATION AND INVESTIGATION slip (form no PK-50-F002) shall be made available to all departmental head by safety department. 2. As soon as the accident takes place the group leader/line engineer/supervisor shall arrange to send injured person to the dispensary for first aid. 3. Line engineer/supervisor shall fill up the ACCIDENT INTIMATION AND INVESTIGATION slip (form no PK-50-F002) in quadruplets with all details shall hand over to medical officer for his remarks. 4. Medical officer shall examine and give treatment to an employee. According to the nature of injury he shall advise an injured person back to work, rest for days, or to go to ESIC/company panel doctor, refer private hospital for further or to come back on so and so date for review. 5. Medical officer will fill up his part in accident report form (form no PK-50-F002) in quadruplets. 6. After signature of medical officer all 4 copies shall be separated. Pink copy for safety dept. and blue for medical dept. shall be kept at dispensary. 7. Other 2 copies yellow copy to concerned departmental head for his records and white copy to injured person for special leave if he is not covered in ESIC. If injured person is under ESIC scheme he can use the same for getting ESIC treatments if any doctor asks the same. 8. The accident form no PK-50-F002 shall include: - exact location of accident, time date, shift etc. - Details of injured person, contact no. age, ESIC no. , Div. etc. - Brief description of accidents. - Cause of injury. - Name of line engineer witness. - Recommendations for continual improvement by HOD. 9. Safety department shall collect their copy from the dispensary and verify the occurrence and the accident and shall complete the loop after investigation with line engineer. 10. Monthly report of all cases shall be sent to the time officer for marking them present on the day of accident. 11. All accidents cases are filled up on line safety system for getting the report of accident free employees and department wise, in jury wise and body part wise. This data is used for communication in safety meetings and taking necessary preventive measures. 12. The data is used to improve PPEs, working conditioned and inputs in safety instruction, training. 13. Project wise and B.C wise man hours are received by safety dept. from PSD. Man days lost are calculated as per date of resuming of injured person. 14. The concerned dept. head shall investigate the accident. 15. Safety coordinator shall verify the accident activity and present list of dept. activity.

ON SITE EMERGENCY MANAGEMENT PLAN


On Site Emergency Plan lay down the code of conduct of all personnel in the premises and the procedures to be adopted by them in the event of an emergency. These procedures have been prepared taking in to account the minimum strength of manpower available at all times in the premises. The responsibilities assigned shall be carried out by the individuals under the direction of the respective team leaders. However, Chief Emergency Controller/Emergency Controller has the authority to approve of deviation and reassign the work for any individual. The emergency procedures outlined are suitable for round the clock coverage, including holidays. These emergency procedures shall be followed as outlined in this On Site Emergency Plan during general shifts as well. The additional manpower available during general shifts will be at the disposal of Chief Emergency Controller/ Emergency Controller. It is suggested that all concerned personnel read these procedures and have the same registered in their mind t ensure orderly action and to ensure the safety of the personnel and plant ATTENDING EMERGENCY: The following are some of the major types of emergency which may be encountered in the industry. 1. FIRE Person who detects fire shall shout FIRE, FIRE and FIRE. He shall inform fire brigade by operating hotline(100,3592,3666), safety department(intercom no. 3677, 3858). Person working at site shall start firefighting operation without delay of time. They will immediately make use of portable fire extinguishers and fire hydrant system. All concern shall carry out their duties as per emergency management plan. Shift in charge shall immediately rush to the site of fire. Sift engineer shall inform utility engineer, electrical engineer and telephone operator/security. In the event of a fire outside the fence the shift in charge shall initiate whatever action is needed to contain the blaze and protect the plant, equipment and building. When fire is extinguished fully after consulting senior manager/officer, emergency leader shall give all clear signals. In case of fire the workers should look toward the nearest wind sock and then should gather at the emergency assembly point which is at perpendicular direction to the wind flow direction.

2. LPG/PROPANE TANK YARD Gas detection system will raise the alarm and water spray nazzle will automatically start in case of any gas leakage. The security gate , fire brigade and safety department should be informed. Supply valves of CNG, propane, LPG must be closed. Start the fire fighting using DCP or CO2 fire extinguisher . Keeping in mind the hazards that can take place, appropriate PPEs must be used. Shut down the plant if required.
