Examination Conditions: The examination was administered by: Dallas Polygraph Services, CentreSuites at HighPoint, 12225 Greenville Ave, Ste 700, Dallas, Texas 75243. Office Phone: (214) 438-3880. Examination was conducted at examiners office. This examination was recorded in part
by audio and video equipment.

Professional Equipment The exam was administered utilizing a DELL LATITUDE lap top Model No. D520, SERIAL NUMBER 373842 with LAFAYETTE LX 4000 Computer Assisted Polygraph software which has the capability of monitoring and recording the following; thoracic and abdominal respiratory activities, galvanic skin resistance (GSR), relative blood pressure, pulse rate, and blood volume of the cardiovascular system. Behavioral Observations & Evaluation of Suitability The above named Examinee reported for the appointment and the following behavioral observations were made during the pre-test interview: Examinee was polite with a positive attitude and straightforward with her answers. Examinee was asked to complete some preliminary administrative forms. Examinee accepted this task in a casual/friendly manner and complied with little or no correction. She completed the forms, agreed to take the polygraph examination and signed all required waivers and releases. This firm (Dallas Polygraph Services) and this Texas licensed polygraph examiner take no position and offer no legal advice in this matter. Validated Test Format Relevant test charts were administered using standardized procedures consistent with validated test formats identified as the Zone Comparison Technique & a 3 Question Federal Zone which enjoys the most current peer reviewed and published research (see American Polygraph Association articles) to confirm the highest reliability and validity in a specific issue examination of this type. Pre-Test Interview Notes and/or Statements made by requestor or examinee Mrs. Bereiter stated that she did not lie to the police about the incident involving Jason when he threatened her son. Nor did she file the report after Jason and his wife apologized for Jason threatening her son.

Relevant Questions with Responses Noted below are the relevant test questions along with the responses noted in parenthesis: 1. Did you lie to the police about Jason threatening your son on the night in question 08/21/2012? (NO) 2. When you filed the report with the Dallas Police Dept. about Jason physically threatening your son, did you lie about anything you said on the report? (NO) 3. Did you file the incident report on Jason after Jason and his wife apologized to you for what Jason did on the night in question? (NO) Test Results: All of the questions on all of the charts were hand scored and found to be well within the range of a non- deceptive examination.
